Fragestellung. Die vorliegende Studie diente der klinischen Evaluation eines vereinfachten Rckatmungsverfahrens zur nichtinvasiven Bestimmung des Herzzeitvolumens (HZV) bei kontrolliert beatmeten Patienten. Methodik. Bei 40 Patienten wurden der effektive pulmonale Blutfluss (PBF) und die Sauerstoffaufnahme ( ) mit einem Rckatmungssystem (AMIS 2001, Innovision, Dnemark) bestimmt. Mit Hilfe einer arteriellen Blutgasanalyse wurden der arterielle (CaO2) und kapillre Sauerstoffgehalt (CcO2) abgeschtzt und der intrapulmonale Shunt berechnet ( ). Aus diesen Parametern wurde das mittels Rckatmung ermittelte bestimmt ( ). Als Referenzmethode wurde die HZV-Messung mittels Thermodilution (HZVthd) herangezogen und als Referenzwert der Mittelwert von HZVthd und HZVrb zugrunde gelegt. Als Referenz der nichtinvasiven Shuntbestimmung diente die Kalkulation aus arterieller und gemischt-venser Blutgasanalyse. Bei 15 Patienten wurde auerdem die Reproduzierbarkeit des neuen Verfahrens getestet. Ergebnisse. HZVthd variierte von 3,79,5l/min (6,11,6l/min; Mittelwert SD). Die mittlere Abweichung zwischen HZVthd und HZVrb betrug 0,18l/min oder 2,9%, die Messgenauigkeit betrug 0,61l/min oder 10,0%. Der intrapulmonale Shunt wurde mit einer Genauigkeit von 2,1% bestimmt. Die Reproduzierbarkeit des Verfahrens betrug 0,24l/min oder 3,9%. Schlussfolgerung. Die hier prsentierte Rckatmungsmethode ermglicht die nichtinvasive Bestimmung des Herzzeitvolumens mit guter Genauigkeit und ausreichender Reproduzierbarkeit. Vor einer Anwendung in der klinischen Routine sind technische Verbesserungen der Methode und weitere Untersuchungen, insbesondere an Patienten mit sehr hohem Herzzeitvolumen und hohen Shuntfraktionen erforderlich.
