Epidermoid carcinomas of the maxillary sinus are often diagnosed in advanced stages. Neoadjuvant treatment is indicated to improve surgery. We report a retrospective study of 30 consecutive cases treated by neoadjuvant chemotherapy based on Cisplatyl® and 5-fluoro-uracil.We estimate the efficiency of this treatment. Patients were classified into T4 (97%) and T3 (3%) groups, and were treated between 2006 and 2008. We achieved complete and partial response in 20% and 33.5% of the cases, respectively. Surgery became possible for 17% of the patients. No grade 4 toxicity was noted. The grade 2 and 3 toxicities were of digestive (28%), hematologic (32%), infectious (15%), renal (7%), and medullar (4%). The recurrence rate was 17% within a median time of six months. Metachronous metastases were found in 12% of cases (cervical nodes and cerebral location) in an average time of 10 months. The mortality was 57% at 18 months. Our results were lower than those of the literature and we suggest to improve care of our patients in the field of surgery and radiotherapy.
