Fragestellung Ziel dieser Untersuchung war es, die Gewebsstoffwechsellage verschiedener Gewebe im hypovolämen Schock und konsekutiver Volumen- bzw. Katecholamintherapie aufzuzeigen, um anhand der In-vivo-Mikrodialyse den Erfolg der Therapie objektivieren zu können.
Material und Methodik Bei 21 Schweinen wurde ein schwerer hypovolämer Schock induziert, indem ihr Blutdruck für 60 min durch Aderlass auf 50% des Ausgangswertes gesenkt wurde. Neben den klinischen systemischen Parametern wurden die durch In-vivo-Mikrodialyse ermittelten Konzentrationsverläufe verschiedener Metaboliten des Gewebsstoffwechsels (Niere, Muskel) ausgewertet. Während des Schockes und der 120-minütigen Phase der therapeutischen Intervention mit Adrenalin bzw. HAES wurden die Skelettmuskulatur und die Niere untersucht.
Ergebnisse Hämodynamisch sind beide Therapieansätze des hämorrhagischen Schockes geeignet, um die Werte der globalen Überwachungsparameter wiederherzustellen. Beide Medikamente scheinen eine lokale Gewebshypoxie zu verhindern. Es gab Anzeichen, dass es unter der Adrenalin-Therapie in der Niere und der Muskulatur zu einer zellulären Schädigung durch den Untergang von Zellmembranen kam. HAES schien derartige Phänomene nicht zu erzeugen.
Schlussfolgerung Weitere Untersuchungen mit Überlebensversuchen sollten darlegen, ob die beobachteten Gewebsschäden mit dem outcome des Schockpatienten korrellieren. Des Weiteren sollte das biochemische Monitoring als wertvolle Ergänzung zur Therapieüberwachung im intensivstationären Kontext erwogen werden.
