Hintergrund
Intradermale Injektionen in der ästhetischen Medizin erfreuen sich in den letzten Jahren einer zunehmenden Nachfrage. Da es sich hierbei um als relativ gering-invasiv und daher auch als relativ komplikationsarm eingestufte Eingriffe handelt, werden sie vermehrt von unterschiedlichsten medizinischen und nichtmedizinischen Disziplinen durchgeführt. Trotz der allgemeinen Verharmlosung dieser Eingriffe sind in der Literatur mehrfach Komplikationen beschrieben worden, die von lokalen Reizzuständen in Form von Rötungen und Schwellungen über Nekrosen bis hin zu schwerwiegenden zerebralen und chorioretinalen Gefäßverschlüssen reichen.
Fallbericht 1
Es wird über eine 41-jährige Patientin berichtet, die unmittelbar nach Hyaluronsäureunterspritzung in die Glabellaregion einen Visusverlust am rechten Auge beklagte. Durch eine Hämodilutionstherapie mit Pentoxifyllin, eine intravenöse Therapie mit Prednisolon und Acetazolamid, eine Lokaltherapie mit Dorzolamid, eine intraokulare Drucksenkung durch Vorderkammerpunktion sowie eine intravitreale Triamcinolon-Injektion konnte ein Visusanstieg von Amaurose auf 0,05 mit exzentrischer Fixation erreicht werden. Als Pathogenese des Ereignisses wird eine zu tiefe (intravasale) Injektion von Hyaluronsäure angenommen, die zu einer retrograden Embolie über die A. ophthalmica in die A. centralis retinae geführt hat.
Fallbericht 2
Es wird über eine 40-jährige Patientin berichtet, die unmittelbar nach der Injektion von autologem Fett in die Glabellaregion über Visusverlust am rechten Auge sowie Hemiparese links klagte. Die daraufhin folgende systemische Lysetherapie führte zu einer Rückbildung der Lähmung im linken Arm bei fehlender Visusbesserung. Als Ursache des Ereignisses wurde ein persistierendes Foramen ovale mit Rechts-links-Shunt nachgewiesen.
Schlußfolgerung
Auch wenn intradermale Injektionen nur selten zu Komplikationen führen, ist aus medizinischer Sicht eine gründliche Aufklärung des Patienten sowohl über geringgradige Komplikationen, z. B. lokale Reizzustände, als auch über schwerwiegende Komplikationen, wie zerebrale oder auch chorioretinale Gefäßverschlüsse, unvermeidbar. Die Durchführung dieser Eingriffe erfordert fachkundiges Wissen über Anatomie und Injektionstechnik. Auch sollte die Bedeutung einer gründlichen Anamnese gerade im Hinblick auf kardiovaskuläre Erkrankungen nicht außer Acht gelassen werden.
