Ziel der Studie: Zu untersuchen, ob eine kontinuierliche, niedrig dosierte Prednisolon-Behandlung die im Röntgenbild erkennbare Gelenkdestruktion bei früher chronischer Polyarthritis verzögert.
Methoden: In einer multizentrischen randomisierten Doppelblindstudie wurden Patienten mit einer frühen aktiven c. P. (Krankheitsdauer < 2 Jahre), die noch nicht mit Gold oder MTX vorbehandelt worden waren, zwei Jahre lang mit 5 mg Prednisolon täglich oder Placebo behandelt. Bei allen Patienten wurde gleichzeitig eine Behandlung mit parenteralem Gold oder Methotrexat eingeleitet. Das Gold konnte bei Auftreten von Nebenwirkungen oder Unwirksamkeit durch MTX ersetzt werden oder umgekehrt. Röntgenbilder der Hände und Vorfüße wurden zu den Zeitpunkten 0, 6, 12 und 24 Monate angefertigt. Alle Bilder wurden von einem Untersucher (S. W.) in Kenntnis der Reihenfolge, aber ohne Kenntnis der Patientenidentität nach dem Ratingen Score und der van der Heijde Modifikation der Sharp-Methode befundet.
Ergebnisse: 196 Patienten wurden eingeschlossen. Die hier berichteten Ergebnisse betreffen 76 Kranke, die die Studie protokollgemäß beendeten. 34 Patienten wurden mit Prednisolon, 42 mit Placebo behandelt. Als Basistherapeutikum hatten 48 anfangs Gold, 28 MTX. 17 Patienten wechselten von Gold auf MTX, einer von MTX auf Gold. Die Mittelwerte der Röntgenscores finden sich in der Tabelle.
Die Progression in der Prednisolon-Gruppe war während des 1. Jahres, insbesondere während der ersten 6 Monate, signifikant geringer als in der Placebo-Gruppe: so stieg der Erosions-Score in der Prednisolon-Gruppe während der ersten beiden Halbjahre um je 0,4% des maximal möglichen Scores, in der Placebo-Gruppe im 1. Halbjahr um 1,8, im zweiten um 0,8%. Während des 2. Jahres war die Progression signifikant geringer (−0,1% in der Prednisolon-Gruppe, 0,2% in der Placebo-Gruppe). Nach 24 Monaten war der Gesamtscore in der Placebo-Gruppe um 2,6%, in der Prednisolon-Gruppe um 1,1% angestiegen.
Diese Ergebnisse der Completer-Analyse wurden durch die Intention-to-treat Analyse (80 Patienten in der Prednisolon-Gruppe, 86 in der Placebo-Gruppe) bestätigt.
Schlussfolgerung: Zusätzlich zur konventionellen Basistherapie mit MTX oder i. m. Gold verzögert eine Prednisolon-Behandlung mit täglich 5 mg die röntgenologisch erkennbare Progression der frühen c. P. signifikant. Die Studie zeigt aber auch, dass nach 6–12 Monaten die Progression unter Basistherapie + Placebo ebenfalls auf minimale Werte reduziert wird.
Die hier dargestellten Ergebnisse der Completer werden durch die Analyse der Intention-to-Treat Population bestätigt.
