Samenvatting
Van Gijn, Van den Berg en Van Lier beschreven een zeldzame hartchirurgische complicatie op een voor de huisarts lezenswaardige wijze (H&W 2003;46:624- 6). Ze vestigen de aandacht op de pulsus paradoxus, die gemakkelijk en snel te meten zou zijn en waarvan de aanwezigheid sterk wijst op een harttamponnade. Omdat ik geen ervaring heb met het vaststellen van dit verschijnsel, las ik met belangstelling de uitleg van de auteurs. Ik begreep dat men spreekt van pulsus paradoxus als er een verschil van meer dan 10 mmHg bestaat tussen de systolische druk, waarbij zo af en toe een toon hoorbaar is, en de systolische druk waarbij álle tonen hoorbaar zijn. Dit ken ik echter als uiting van boezemfibrilleren, een differentiële diagnose die zij niet noemen.
