Riassunto Obiettivo
Il nostro obiettivo è stato valutare il carico aterosclerotico coronarico mediante tomografia computerizzata multistrato (TCMS) in pazienti con angina pectoris stabile e instabile.
Materiali e metodi
Ventuno pazienti con angina stabile e 20 pazienti con angina instabile, in attesa di eseguire l’angiografia coronarica convenzionale (ACC), sono stati sottoposti ad angiografia coronarica con TCMS a 64 strati caratterizzata da un tempo di rotazione veloce (330 ms) e da una più elevata emissione del tubo radiogeno (900 mAs). Nel determinare il carico aterosclerotico coronarico alla TCMS, abbiamo valutato l’estensione (numero di segmenti malati), le dimensioni (piccola o estesa), il tipo (calcifica, non calcifica o mista) di placca, la sua distribuzione anatomica e l’aspetto angiografico in tutti i segmenti disponibili ≥2 mm. In un sottogruppo di 15 pazienti (7 con angina stabile e 8 con angina instabile) il rilevamento e la classificazione delle placche coronariche mediante TCMS sono stati verificati mediante ecografia intra-coronarica (ICUS).
Risultati
La sensibilità e la specificità della TCMS comparata all’ICUS nell’identificare placche significative (definite come placche con spessore 31 mm all’ICUS) sono state dell’83% e 87%. Complessivamente sono stati inclusi 473 segmenti, risultando in una media di 11,6±1,5 segmenti per paziente. Le placche erano presenti nel 62% dei segmenti e sono state classificate come estese nel 47% dei segmenti malati. Il 32% delle placche erano non calcifiche, il 25% calcifiche e il 43% miste. Le placche erano localizzate più frequentemente a livello dei segmenti prossimali e medi. È stata riscontrata la presenza di placche nel 33% dei segmenti classificati come normali all’ACC. I pazienti con angina instabile avevano una prevalenza significativamente maggiore di placche non calcifiche a confronto con i pazienti con angina stabile (45% vs. 21%, p<0,05).
Conclusioni
L’angiografia coronarica mediante TCMS fornisce importanti informazioni riguardo il carico aterosclerotico coronarico nei pazienti con angina stabile ed instabile.
