Résumé Objectif
La région Nord-Pas-de-Calais présente des taux d’allaitement maternel parmi les plus bas de France, mais l’implication des maternités dans démarche « Initiative hôpital ami des bébés » (IHAB) s’est récemment développée dans plusieurs établissements. Elle est soutenue par les quatre réseaux régionaux de santé en périnatalité. L’objectif de cette étude est d’évaluer les pratiques en matière d’allaitement maternel dans la région en regard des conditions IHAB et d’appréhender la perception du label par les professionnels.
Méthode
Une enquête de pratiques a été réalisée courant mai 2009 dans les 35 maternités de la région. Le questionnaire établi à partir du formulaire d’autoévaluation de l’OMS/Unicef a été envoyé aux responsables des différents établissements.
Résultats
Trente-quatre maternités sur 35 ont répondu. Les taux d’allaitement obtenus sont hétérogènes et s’échelonnent de 36 à 75 % à la naissance et de 24 à 72 % à la sortie de maternité. À la sortie de maternité, 12 maternités seulement présentent un taux supérieur à 50 %. Cette hétérogénéité est constatée également dans les pratiques en matière d’accueil du couple mère-enfant comme dans l’information et le soutien à l’allaitement maternel. Les conditions 4 (peau à peau), 5 et 8 (aide à l’allaitement maternel) sont respectées par plus de 23 établissements. Les conditions 9 et 6, relatives aux dons de compléments et à la tétine, sont citées respectivement par 13 et 10 maternités comme les plus difficiles à mettre en place. Treize maternités ont un taux de formation du personnel soignant supérieur ou égal à 80 %. Les habitudes professionnelles de ces établissements sont en meilleure adéquation avec les recommandations du label, et la diminution relative du taux d’allaitement maternel entre la naissance et la sortie y est également significativement plus faible (−6,5 versus −13,4 %, p = 0,04). Dix-neuf maternités sont engagées ou souhaiteraient s’engager dans la démarche IHAB.
Conclusion
Le manque d’harmonisation des pratiques et les arrêts précoces d’allaitement en maternité semblent pouvoir être diminués par une meilleure formation des soignants. Cette analyse régionale témoigne du grand intérêt des professionnels de la périnatalité pour la démarche IHAB.
