Distale Humerusfrakturen stellen sowohl bei Kindern als auch bei Erwachsenen Problemfrakturen dar. Während sie bei Ersteren sehr häufig sind und v. a. suprakondylär extraartikulär auftreten, handelt es sich bei Erwachsenen um seltene, überwiegend intraartikuläre Frakturen. Gute Ergebnisse sind sowohl bei Kindern als auch bei Erwachsenen nur unter Beachtung der komplexen Anatomie, der Knochenqualität, der Begleitverletzungen, der Biomechanik der Verletzung und der daraus abzuleitenden Operationstechniken sowie einer gut strukturierten Nachbehandlung zu erzielen. Besondere Beachtung muss den häufigen Begleitverletzungen und den oft auftretenden intra- und postoperativen Komplikationen gewidmet werden. Es wird ein Überblick über die Epidemiologie, die aktuellen Diagnose- und Therapiestandards, die möglichen Komplikationen sowie die zu erwartenden Ergebnisse gegeben.
