Vorgestellt wird der Fall eines im Alter von 59 Jahren verstorbenen Mannes, der mit 29 Jahren 1 Jahr als politischer Gefangener inhaftiert war. Eine chronisch-progrediente Glomerulonephritis hatte sich als Folge der extremen Haftbedingungen verschlechtert und zu einer hämodialysepflichtigen Niereninsuffizienz geführt. Nach bilateraler Nephrektomie war eine einseitige Nierentransplantation erfolgt. Im Verlauf der begleitenden immunsuppressiven Therapie hatten sich mehrere Posttransplantationsneoplasien in Gestalt eines Adenokarzinoms der Harnblase und multipler, metachron entstandener prä-/neoplastischer Hautveränderungen entwickelt. Sechs Monate vor dem Tod und 27 Jahre nach Beginn der Immunsuppression war an der linken Ohrhelix ein Merkel-Zell-Karzinom aufgetreten, das chirurgisch in sano entfernt wurde. Der Tod ist im tumortoxischen Multiorganversagen bei ausgedehnter hämatogener Metastasierung des Merkel-Zell-Karzinoms eingetreten. Die zum Tod führende Tumorkrankheit ist als mittelbare Folge der extremen Haftbedingungen anzusehen.
