Operationsziel
Achsgerechte Implantation einer Endoprothese des Kniegelenks mit symmetrischem Gelenkspalt über den gesamten Bewegungsumfang sowie Rekonstruktion der Gelenklinie durch Verwendung bildfreier Computernavigation.
Indikationen
Fortgeschrittene symptomatische Gonarthrose nach erfolgloser konservativer und gelenkerhaltender operativer Therapie.
Kontraindikationen
Floride Infektionen, Weichteildefekte und hochgradige Seitenbandinstabilitäten.
Operationstechnik
Medialer parapatellarer Zugang zum Kniegelenk, Patellaverschmälerung, Anbringen von Referenzsternen an Tibia und Femur. Erfassen von Knochenoberflächen und Bandspannungen des Kniegelenks durch das Navigationssystem. Achsgerechte tibiale Resektion. Ligamentbalancing in Streckung zur Wiederherstellung des symmetrischen Streckspalts. Erhalt von Gelenklinie und Optimierung des Verhältnisses zwischen Streck- und Beugespalt in der femoralen Navigationsplanung. Achsgerechte distale femorale Resektion. Rekonstruktion des symmetrischen Beugespalts durch Rotation der Femurkomponente parallel zur tibialen Resektion. Sägeschnitte und Probereposition. Zweizeitiges Zementieren der Prothese. Überprüfen der Bandspannungen über den gesamten Bewegungsumfang und der mechanischen Achse. Schichtweiser Wundverschluss.
Weiterbehandlung
Intensive Physiotherapie und Motorschienenbehandlung. Mobilisation mit 20 kg Teilbelastung für 2 Wochen, anschließend mit beschwerdeadaptierter Vollbelastung an Unterarmgehstützen für weitere 4 Wochen.
Ergebnisse
Die Analyse von 582 aufeinanderfolgenden navigierten Knieprothesenimplantation zeigte eine Streckspaltinstabilität > 3 mm in einem Fall (0,2%) und eine Beugespaltinstabilität > 3 mm in 8 Fällen (1,4%). Ein zu enger Beugespalt resultierte in 23 Fällen (4,4%), ein zu weiter Beugespalt in 13 Fällen (2,5%). Die Gelenklinie wurde durchschnittlich um 0 mm angehoben, bei 17 Patienten um > 3 mm verlagert (2,9%).
