Ein 36jhriger Mann erlitt nach einem Fahrradsturz ein isoliertes Schdel-Hirn-Trauma (SHT) mit Schdelbasisfraktur, Epidural- und Subduralhmatom sowie einer Kontusionsblutung [Abbreviated Injury Scale (AIS): 5 Punkte]. Die raumfordernden Hmatome wurden durch Kraniotomie evakuiert. Hohe ICP-Werte machten ein Barbituratkoma ber 10 Tage notwendig. Erschwerend entwickelte der Patient am 8. Tag nach Trauma ein septisches Lungenversagen. Der Patient zeigte einen protrahierten Verlauf und konnte erst 57 Tage nach dem Unfall in die Neurorehabilitation verlegt werden. In der Folgezeit entwickelte der Patient eine massive Einschrnkung der Beweglichkeit in beiden Hftgelenken (Extension/Flexion: rechts: 0/10/20, links: 0/10/30; Auenrotation/Innenrotation: rechts und links: 5/0/5; Abduktion/ Adduktion: rechts: 10/0/25, links: 10/0/10). In der Beckenbersichtsaufnahme und der Computertomographie zeigten sich ankylosierende heterotope Ossifikationen (HO) beider Hftgelenke (Brooker Typ IV). Die HO wurden 2 Jahre nach dem Unfall zweizeitig jeweils ber einen Smith-Petersen-Zugang entfernt. Die HO-Rezidivprophylaxe erfolgte mittels einer einmaligen properativen Bestrahlung (8 Gy) und einer postoperativen Verabreichung von Indomethacin (150 mg/Tag) ber 3 Monate. Die Bewegungsausmasse an den Hftgelenken sind 15 (links) bzw. 12 (rechts) Monate nach Resektion wie folgt: Extension/Flexion: rechts: 5/0/90, links: 5/0/100; Auenrotation/Innenrotation: rechts: 20/0/30, links: 20/0/20; Abduktion/Adduktion: rechts: 30/0/40, links: 30/0/40. Radiologisch zeigte sich bisher kein Rezidiv der HO. Der Patient kann 3 Jahre nach dem Unfall leichten Ttigkeiten als Landwirt nachgehen.
A 36-year old man suffered an isolated head injury with a fracture of the skull, epidural and subdural hematomas as well as brain contusion (Abbreviated Injury Scale (AIS): 5 points). The hematomas were evacuated by craniotomy. Because of high intracranial pressure (ICP) a barbiturate coma was necessary. Additionally, the patient demonstrated acute lung injury (ARDS) due to pneumonia 8 days after trauma. The patient recovered slowly and was transferred to neurorehabilitation on day 57 after injury. During the following 12 months the patient developed a significant deficit of hip motion (extension/flexion: right: 0/10/20, left: 0/10/30; external/internal rotation: right and left: 5/0/5; abduction/adduction: right: 10/0/25, left: 10/0/10). X-rays and CT-scanning revealed severe heterotopic ossification (HO) of both hips with ancylosis (Brooker type IV). Resection of HO was carried out in a two stage procedure using Smith-Petersen approach. The prophylaxis for recurrence of HO included preoperative single-dose radiation (8 Gy) and postoperative treatment with indomethacin (150 mg per day). The patient revealed 15 (left hip) and 12 (right hip) months after surgery the following range of hip movement: extension/flexion: right: 5/0/90, left: 5/0/100; external/internal rotation: right: 20/0/30, left: 20/0/20; abduction/adduction: right: 30/0/40, left: 30/0/40. No recurrence of HO was observed in x-ray. The patient is able to work in his profession as farmer.
