Evidenzbasierte Aussagen zur Frage, ob das Poplitealarterienaneurysma (PAA) endovaskulär oder besser offen-chirurgisch angegangen werden sollte, fehlen. Immerhin gibt es aber keine größeren Sicherheitsbedenken gegen das endovaskuläre Vorgehen, sodass in einigen europäischen Ländern Datenbanken eingerichtet wurden, die die Ergebnisse der Behandlung des PAA dokumentieren. Die vorliegenden Ausführungen begründen, warum eine solche Registratur auch bei uns vorgehalten werden sollte. Erörtert wird auch die Alternative, eine prospektiv randomisierte Studie, wie sie mittlerweile für asymptomatische Patienten unter der Clinical Trials Nummer NCT01817660 in den USA initiiert wurde. Diese Alternative wird aufgrund der zu erwartenden langen Rekrutierungszeit bei der Seltenheit der Erkrankung abgelehnt. Außerdem sprechen die zahlreichen Ausschlusskriterien und notwendigen Stratifizierungen einer randomisierten Studie gegen ein solches Vorhaben. Vielmehr muss es die Aufgabe sein, die Ergebnisse flächendeckend bei sämtlichen Formen des PAA (symptomatisch/asymptomatisch/Thrombose als Notfall) bekannt zu machen und die Amputationsraten zu erfassen. Eine Register-Konzeption wird vorgelegt.
