Operationsziel
Deckung von Weichteildefekten mit freiliegendem Knochen, Sehnen oder Osteosynthesematerial an der oberen und unteren Extremität und im Kopf-Hals-Bereich durch Transplantation einer freien, mikrovaskulär angeschlossenen Lappenplastik.
Indikationen
Weichteildefekte an den Extremitäten und in der Kopf-Hals-Region bis zu einer Größe von 42 × 15 cm.
Kontraindikationen
Traumatisierung an den anterolateralen Oberschenkeln durch Unfall oder Voroperationen. Fehlende technische oder personelle Ressourcen.
Operationstechnik
Zunächst Umschneiden der Hautinsel medial über dem Musculus rectus nach präoperativer Markierung der Lappenachse auf einer Linie von der Spina iliaca anterior superior zum superolateralen Patellapol. Aufsuchen und Präparation der Perforatoren des Ramus descendens der Arteria circumflexa femoris lateralis bis zu ihrem Stammgefäß und Präparation derselben, bis die gewünschte Stiellänge erreicht ist. Dann Umschneiden der Hautinsel lateral. Danach Absetzen des Lappens und mikrochirurgischer Anschluss sowie Einnähen in der Empfängerregion.
Weiterbehandlung
Klinische Lappenkontrolle und/oder Lappenmonitoring für 1 Woche. Bei Lappentransfer an die Extremitäten striktes Hochlagern der betroffenen Extremität. Ist die untere Extremität betroffen, Bettruhe für 1 Woche. Thromboseprophylaxe.
Ergebnisse
In den Jahren 2008 bis 2011 wurden an unserer Abteilung insgesamt 41 freie anterolaterale Oberschenkellappen an 5 weiblichen und 36 männlichen Patienten mit einem Durchschnittsalter von 53 Jahren (38–70 Jahre) zur mikrochirurgischen Weichteilrekonstruktion verwendet. Die Lappenverlustrate betrug insgesamt 2,4 %, wobei revisionspflichtige Komplikationen wie Hämatom, Probleme der mikrochirurgischen Anastomose oder Wundheilungsstörung bei insgesamt 13,8 % der Patienten auftraten.
