Hintergrund. Die Pierre-Robin-Sequenz ist ein Fehlbildungskomplex mit Mikrogenie, Unterkieferrcklage und Glossoptose verbunden mit einer Zungenrcklage: Die Zunge ragt in den Nasen-Rachen-Raum hinein und verlegt partiell den Larynxeingang. Dies fhrt besonders in der Neugeborenenphase zu Atemstrungen, die zu Risiken werden knnen. In letzter Zeit wurde die Distraktionsosteogenese als ein viel versprechendes Therapiemittel eingefhrt. Wir haben dieses Konzept fr Neugeborene bis 3Monate berarbeitet. Patienten. Der 1. Patient wurde im Alter von etwa 3Monaten operiert, und es wurde eine beidseitige Unterkieferdistraktion mit einem extraoralen Distraktor durchgefhrt. Der 2. Patient wurde frher operiert, und zwar im Alter von etwa 2Monaten. Es wurde ein neuer Distraktor verwendet, der von unserer Klinik spezifisch fr Neugeborene konzipiert worden ist. Ergebnisse. Die Distraktion verlief in beiden Fllen erfolgreich und viel versprechend. Nach 20Tagen atmete der erste Sugling ohne die Trachealkanle. Im 2. Fall konnte der Wendl-Tubus bereits am 7. postoperativen Tag entfernt werden. Der Lngengewinn betrug im 1. Fall 20mm und im 2. Fall 15mm beidseitig. Diskussion. Die blichen Unterkieferdistraktoren weisen bei Suglingen viele Nachteile auf, u.a. Narbenbildung an den Pindurchtrittstellen und frhzeitiger Pinsverlust. Aus den oben genannten Grnden wurde von uns eine neue Art von Distraktor entwickelt, der fr diese Probleme eine Lsung mit sich bringt. Weniger Raumbedarf, Pinabwesenheit mit dementsprechender Verkleinerung von entstellenden Narben und gnstiger Bettigungshebel sind die klinischen Vorteile dieses Distraktors.
Background. The prognosis for Pierre Robin sequence is often closely correlated with direct postnatal asphyxia (ARDS) and permanent cerebral damage. Unfortunately, all of the well-known treatments entail many problems, which often lead to medical care of the patient for many years. Distraction osteogenesis was recently introduced as a very promising procedure. We revised this concept for newborn children aged up to 3months. Materials and methods. The first patient was operated with an extraoral distractor at an age of approx. 3months. The second patient was operated at an age of approx. 2months. A new distractor was used, conceived specifically by our department for newborn children. Results. Distraction osteogenesis was successful in both cases. After 20days the first patient was able to breathe without the tracheotomy cannula. In the second case, we removed the nasopharyngeal tube successfully on the 7th postoperative day. The bilateral length gain amounted to 20mm in the first case and 15mm in the second. Discussion. The usual jaw distraction osteogenesis has many disadvantages in babies: scar formation in the places depressed by pins and early pin loss. For the above-mentioned reasons, we developed a new type of extraoral distractor, which solves these problems. Less required space and the absence of pins resulting in fewer disfiguring scars are the clinical advantages of this new distractor.
