Gonadale Keimzelltumoren werden heute standardisiert behandelt. Die Cisplatin-haltige Chemotherapie und multimodale Therapieverfahren haben die Heilungsrate enorm verbessert [7]. Primärtumoren werden fast immer operativ behandelt. Nach der seltenen primären Chemotherapie müssen verbliebene Tumorreste möglichst genau entsprechend der WHO-Klassifikation eingeteilt werden. Ein verbindliches System zur Dokumentation der Tumorregression fehlt. Auch nach der seltenen diagnostischen, retroperitonealen Lymphadenektomie nach Ablatio testis erfolgt die Klassifikation nach der WHO- und der TNM-Klassifikation. Problematisch ist die Untersuchung der Präparate einer retroperitonealen Lymphadenektomie nach Chemotherapie. Die Morphologie ist oft bizarr, präparatorische und terminologische Standards fehlen. Ist noch ein vitaler Tumor vorhanden, dann handelt es sich meist um ein Teratom, bei dem eine „reife“ und eine „unreife“ Form unterschieden werden. Enthält ein Präparat andere Differenzierungen, erfolgt die Klassifikation nach der WHO-Klassifikation der Keimzelltumoren. Sarkome oder Karzinome müssen zuverlässig abgegrenzt und klassifiziert werden, da sie andere therapeutische Konsequenzen haben als Keimzelltumoren. Die Terminologie muss zwischen Pathologie und Klinik abgesprochen werden, da verbindliche Definitionen fehlen.
