GRUNDLAGEN: Auf kaum einem anderen Gebiet maligner Tumore hat sich in den letzten Jahrzehnten das Verständnis betreffend Ätiologie, Epidemiologie, aber auch der diagnostischen und therapeutischen Maßnahmen, so stark verändert wie beim Plattenepithelkarzinom des Analkanals. METHODIK: Es wird die derzeitige Sicht der Behandlungsstrategie nach einem systematischen Review der Literatur klar dargestellt. ERGEBNISSE: Das humane Papillomavirus (HPV) steht heute als Ursache eigentlich außer Diskussion. Die Risikofaktoren für ein Analkarzinom beinhalten daher auch die Risikofaktoren für eine anale HPV-Infektion. Zusätzlich spielen Noxen (Zigarettenrauchen) und immunologische Faktoren eine Rolle (HIV-Infektion, Immunsuppression). Auf dem Gebiet der Diagnostik hat durch die hochauflösende Anoskopie (Äquivalent der Kolposkopie) eine potente Erweiterung der Früherkennung stattgefunden, aber auch die Therapie hat eine Verfeinerung ihrer Möglichkeiten erfahren. Als Therapiestandard hat die kombinierte Radio/Chemotherapie der Chirurgie durch exzellente Ergebnisse speziell in Bezug auf kolostomiefreies Überleben längst den Rang abgelaufen und der früher standardmäßig durchgeführten abdomino-perinealen Exstirpation bleibt heute nur noch der Stellenwert einer "ultima ratio". SCHLUSSFOLGERUNGEN: Vorsorgeprogramme für Risikogruppen sind als sinnvoll anzusehen und werden als kosteneffektiv beschrieben, allerdings mangelt es zumindest im europäischen Raum noch an deren Umsetzung. In näherer Zukunft ist durch die Zunahme der RisikopatientInnen auch mit einer Zunahme des Analkarzinoms zu rechnen. Langfristig dürfte für das Analkarzinom durch die Einführung einer HPV-Impfung – wahrscheinlich im Jahr 2006 – ein Rückgang des Auftretens zu erwarten sein.
