58 Patienten nach offenen Unterschenkelfrakturen mit ausgedehntem Hautweichteildefekt, welche interdisziplinär mit Osteosynthesen und freien, mikrovaskulären Lappenplastiken versorgt worden waren, wurden durchschnittlich 6 Jahre nach der Behandlung im Hinblick auf ihre Lebenszufriedenheit untersucht. Verwendet wurde hierzu ein standardisiertes Testverfahren (FLZ) mit nachgewiesener Reliabilität und Validität, sowie ein Fragebogen mit spezifischen Fragen zur Zufriedenheit mit dem Behandlungsergebnis, zum Lokalbefund und dessen Auswirkungen, zu Beruf und ökonomischen Aspekten. In einzelnen Bereichen fanden sich signifikante Diskrepanzen zu einem repräsentativen Vergleichskollektiv aus der BRD-West (z. B. Freizeitgestaltung, Einkommen, Arbeit, Partnerschaft, körperliche Leistungsfähigkeit, Fortbewegungsfähigkeit, Beschwerdefreiheit). Die berechneten, globalen „Lebenszufriedenheitsindices“ der untersuchten Patienten wichen statistisch schwach signifikant vom Referenzwert des Vergleichskollektivs ab. Obwohl spezifische Aspekte (z. B. Aussehen des Beins, Hebedefektmorbidität, Belastbarkeit, Schmerzen) langfristig durchaus kritisch beurteilt wurden, zeigten sich jedoch 91 % der Patienten mit der Behandlung zufrieden. Kein einziger Patient hätte retrospektiv eine Amputation bevorzugt! 64 % der Patienten arbeiteten nach abgeschlossener Behandlung wieder. Die multimodale Behandlung von Unterschenkelfrakturen mit begleitenden Hautweichteildefekten durch ein erfahrenes, interdisziplinäres „Traumateam“, kann zufriedenstellende Langzeitergebnisse liefern. Diese Behandlungsergebnisse werden von der überwiegenden Mehrzahl der Patienten positiv beurteilt. Mit einer Beeinträchtigung der Lebenszufriedenheit zumindest in bestimmten Bereichen muß allerdings gerechnet werden. Die vorliegende Studie gibt wertvolle Informationen sowohl für differentialtherapeutische Überlegungen als auch für das Gespräch mit den Patienten.
