Zusammenfassung Das langfristige Offenbleiben eines Bypasses ist die Voraussetzung fr ein gutes Operationsergebnis in der Koronarchirurgie. Diese Arbeit sollte den Einfluss des Koronararteriendurchmessers auf den intraoperativen Bypass-Fluss und auf die postoperative Bypass-Offenheitsrate analysieren. In der untersuchten Gruppe von 107 KHK-Patienten, bei denen eine ACVB-Operation durchgefhrt wurde, waren 95 (88,8%) Mnner und 12 (11,2%) Frauen mit einem Durchschnittsalter von 52,97,7 (3172) Jahren. Der Innendurchmesser der anastomosierten Arterie und der Fluss durch die angelegten Bypsse wurden intraoperativ gemessen. Eine Kontrollangiographie erfolgte zwischen dem 1. und dem 12. postoperativen Monat (Mittelwert 9,22,7). Insgesamt wurden 373 Venenbypsse untersucht, wovon 298 zum Zeitpunkt der Kontrollangiographie offen waren (Offenheitsrate von 79,9%). Die Werte des Innendurchmessers wurden mit dem Bypass-Fluss und mit der Offenheitsrate verglichen. Es bestand eine deutliche Korrelation zwischen dem Innendurchmesser der Arterie und dem Bypass-Fluss (r=0,768, p0,001) als auch der Offenheitsrate (2,0mm 95,7%; 1,52,0mm 82,3%; 1,5mm 55,5%, p0,05).
Summary Aortocoronary graft patency is essential for postoperative outcome. The aim of the study was to define the influence of coronary artery diameter on blood flow of the performed graft and its patency. 107 patients (95 males and 12 females, mean age 52.97.7 years) who underwent CABG only with saphenous vein grafts and who accepted the postoperative angiographic follow-up were involved in the study. The internal diameter of every grafted artery was intraoperatively measured with the probe (calibrated in mm), inserted in the vessel after its incision. The electromagnetic graft flow measurement was performed after weaning from cardiopulmonary bypass. The patients were angiographically studied up to twelve months (mean 9.22.7) after the surgical procedure. Of 373 performed grafts 298 remained patent (graft patency rate GPR 79.9%). The values of coronary artery diameter were compared with graft blood flow and angiographic data. A statistically significant relationship between artery diameter and GPR was detected (2.0mm 95.7%; 1.52.0mm 82.3%; 1.5mm 55.5%, p0.05), as well as between artery diameter and graft blood flow (r=0.768, p0.001).We conclude that both graft blood flow and patency rate are influenced by the diameter of anastomosed coronary artery.
