Klinische Krebsregister bauen relevante interdisziplinär und sektorenübergreifend verfügbare Befunde und Behandlungen mit systematischem Follow-up zu Krankheitsverläufen zusammen. Auf die Daten ihrer Patienten können Ärzte versorgungsbegleitend wie im Krankenhausinformationssystem zugreifen. Große Einzugsgebiete, flächendeckende Erhebung und valide authentifizierte Daten sind Basis für vielseitige Auswertungen. Im Sinne der Versorgungsforschung können damit Versorgungsqualität und Veränderungen der Langzeitergebnisse gezeigt werden, die durch Gesetzgebung und Finanzierung, durch diagnostische und therapeutische Innovationen und durch Änderungen der Versorgungsstrukturen wie Zentrumsbildungen entstehen können oder sollen. Generieren von Wissen ist dabei ein bisher vernachlässigter Zweck. Diese Perspektiven reichen allerdings nicht aus, die mit Positivbegriffen beschriebene „überbordende“ Medizinalbürokratie einzudämmen. Der Onkologiestandort Deutschland wird so weiter marginalisiert. Deshalb werden hier mantragleich die seit Jahrzehnten bekannten Chancen und die Notwendigkeit dargelegt, mit modernen klinischen Krebsregistern die ökonomischen, beruflichen, sozialen und therapeutischen Entwicklungen zu begleiten und der Onkologie in Deutschland einen Impuls zu geben.
