Die vorliegende bersicht skizziert Pathophysiologie, Diagnostik und Therapie der vensen Thrombembolie unter Beachtung der Besonderheiten in Gynkologie und Geburtshilfe. So wird das familire Thromboserisiko der Patientin durch vorbergehende Risikofaktoren wie Operation, Schwangerschaft oder Wochenbett multiplikativ erhht. Bei Verdacht der tiefen Beinvenenthrombose wird in der bildgebenden Diagnostik die Phlebographie zunehmend von der Sonographie verdrngt. Als wegweisender Schritt zur Diagnostik der Lungenembolie dient der D-Dimertest. Wegen der Bedeutung der Diagnose muss der diagnostische Weg mit weiteren bildgebenden Verfahren jedoch auch bei der Schwangeren zu Ende gegangen werden. Die wesentliche therapeutische Manahme besteht in der Antikoagulation, der sich je nach Befund und Rezidivrisiko eine unterschiedlich lange Rezidivprophylaxe anschliet.
