Die Faktoren, die das berleben der Patienten nach notfallmiger sowie elektiver Operation des AAA signifikant beeinflussen, sind in ihrer Wertigkeit Gegenstand der Diskussion. Anhand einer multivariaten Analyse erfolgte eine Untersuchung von Prognosefaktoren bei 528Patienten mit rupturiertem und nichtrupturiertem AAA, die whrend eines 20-Jahres-Zeitraums operiert wurden. Als unabhngige signifikante Prognosefaktoren fr das rupturierte AAA fanden sich der properative Schockindex, der ASA-Score, die intraoperative Gabe von Aprotinin sowie die Abklemmzeit der Aorta. Fr elektiv operierte AAA lieen sich retrospektiv keine unabhngigen signifikanten Faktoren evaluieren. Nach unserer Erfahrung kann die Komplikationsrate und Frhletalitt beim rupturierten AAA durch Vermeidung eines properativen Schockzustands, kurze Abklemmzeiten der Aorta und im Falle substitutionsrefraktrer Gerinnungsstrungen durch die intraoperative Gabe von hochdosiertem Aprotinin gesenkt werden. Nicht zu beeinflussen sind dagegen vorbestehende Risikofaktoren seitens des Patienten, die den Krankheitsverlauf entscheidend mitbestimmen knnen.
The significance of factors influencing the perioperative mortality of elective and urgent aneurysm repair are discussed. Prognostic factors affecting survival were analyzed in 528 patients with ruptured and unruptured AAA who were operated on within a 20-year period. In patients with ruptured AAA, multivariate analysis revealed that preoperative shock index, ASA score, intraoperative administration of aprotinin, and clamping time of the aorta are predictive of extended survival. No independent factors could be found in patients with unruptured AAA. In our experience, early complications and mortality in patients with ruptured AAA may be reduced by avoiding circulatory shock preoperatively, shortening clamping time, and in cases of clotting impairment refractory to substitution by high-dose administration of aprotinin intraoperatively. Nonetheless, preoperative risk factors of these patients affect the postoperative course that cannot be influenced by the surgeon.
