Background. Damage to peripheral pectoralis major muscle tendon is a rare type of musculoskeletal system injury. So far two cases of the simultaneous, bilateral and total rupture of pectoralis major tendon have been noted. Material and methods. Based on the description of the 31 year old body builder’s case, the authors present the methods of diagnosing and treatment of such injuries. Results. After the clinical assessment and additional tests (x-ray, ultrasound), the patient was qualified for surgical reconstruction of distal pectoralis major tendon. The surgery was performed using titanium anchors. After postoperative rehab and a three month period the patient returned to strength training. Conclusions. The increased popularity of body building using high loads has resulted in a higher prevalence of attachment injuries in such muscles as pectoralis major. So far, however, the reference sources lack case reports on simultaneous bilateral peripheral rupture of pectoralis major tendon and its surgical correction. In the reported case, the initial diagnosis revealed a typical muscle function asymmetry and a typical change in picture of the thorax. The surgical procedure was in this case the treatment of choice.
