En raison de la nature progressive du diabète de type 2 (DT2), la plupart des patients ont auront besoin d'une intensification du traitement. Cet essai de phase 3 vise à étudier l'efficacité et la tolérance de IDegLira dans le contrôle glycémique chez les patients DT2 insuffisamment contrôlés avec un agoniste du récepteur de glucagon (GLP-1RA) et des antidiabétiques oraux (ADO).Dans cet essai en ouvert de 26 semaines, parmi les DT2 insuffisamment contrôlés par une dose maximale de GLP-1RA (liraglutide une fois par jour ou exénatide deux fois par jour) + metformine±pioglitazone±sulfamide, 2 tiers des patients randomisés ont switché pour IDegLira (n=292) et les autres ont continué le traitement GLP-1RA précédent (n=146) ; les ADO précédents ont été conservés dans tous les groupes.Le taux d'HbA1c moyen a diminué depuis l'inclusion dans les deux groupes de traitement (différence estimée entre les groupes IDegLira et GLP-1RA : − 0,94 %, p<0,001). 75 % des patients traités par IDegLira ont atteint l'objectif d'HbA1c < 7 % contre 36 % des patients traités par GLP-1RA (p<0,001) ; 63 % des patients traités par IDegLira contre 23 % par GLP-1RA ont atteint l'objectif d'HbA1c≤6,5 % (p<0,001). L'évolution du poids était de +2,0 kg sous IDegLira et − 0,8kg sous GLP-1RA. La fréquence d'hypoglycémies confirmées était plus élevée avec IDegLira vs GLP-1RA. La dose moyenne de IDegLira à 26 semaines était de 43 doses unitaires (soit 43U d'insuline degludec, 1,55mg de liraglutide).IDegLira permet un meilleur contrôle glycémique par rapport au traitement par GLP-1RA et représente une approche efficace pour intensifier le traitement des patients diabétiques de type 2 insuffisamment contrôlés par GLP-1RA.
