Introduction. - Le syndrome de Tolosa-Hunt est caracterise par une ophtalmoplegie douloureuse recurrente, uni- ou bilaterale, touchant un ou plusieurs nerfs oculomoteurs.Il est secondaire a une infiltration granulomateuse non specifique des sinus caverneux. L'evolution est souvent spectaculaire sous corticotherapie. Les lymphomes touchant les sinus caverneux peuvent simuler un syndrome de Tolosa-Hunt. Les meningiomes et les tumeurs orbitaires sont les autres diagnostics differentiels.Exegese. - Nous rapportons le cas d'un patient de 78<space>ans chez lequel un lymphome non hodgkinien systemique avec atteinte du sinus caverneux a ete revele par une ophtalmoplegie douloureuse evoquant un syndrome de Tolosa-Hunt.Conclusion. - Devant un tableau clinique evoquant un syndrome de Tolosa-Hunt, un bilan exhaustif est imperatif pour eliminer une localisation d'un lymphome non hodgkinien.
Introduction. - Tolosa-Hunt syndrome is characterized by painful ophtalmoplegia due to idiopathic granulomatous inflammation of the cavernous sinus. Steroid therapy dramatically reverses the symptoms and clinical signs. Because they also may respond to steroids, tumors such as lymphoma and meningioma and orbital tumors can make differential diagnosis difficult.Exegesis. - We report the case of a 78-year-old male patient in whom systemic lymphoma associated with inflammation of the cavernous sinus was uncovered by painful, gradually progressing, ophtalmoplegia mimicking Tolosa-Hunt syndrome.Conclusion. - When faced with a clinical picture suggestive of the existence of Tolosa-Hunt syndrome clinical workup is mandatory and should lead to diagnosis of exclusion.
