La sarcoïdose est une maladie granulomateuse systémique de cause inconnue ayant pour localisations privilégiées le poumon et le système lymphatique. La sarcoïdose donne de multiples présentations phénotypiques en fonction du nombre et de la localisation des atteintes viscérales. Certaines présentations sont typiques et fréquentes et leur diagnostic est facile à porter. Il en va différemment avec les formes atypiques, soit du fait de leur rareté, soit du fait de leur présentation trompeuse, que cette présentation trompeuse concerne l’ensemble des manifestations ou l’une d’entre elles. Le diagnostic doit être particulièrement rigoureux pour éviter les erreurs par excès et par défaut.La sarcoïdose comporte de multiples présentations atypiques et, si chacune d’entre elles est peu fréquente, l’ensemble des présentations atypiques regroupe au total 20 % des cas. Les atypies concernent les manifestations pulmonaires et extrapulmonaires, les signes généraux, la biologie et l’histopathologie.Le recensement des formes à présentation atypique de la sarcoïdose doit aider à évoquer leur diagnostic. Le diagnostic repose sur la compatibilité de la présentation clinicoradiologique avec les descriptions recensées, la mise en évidence de lésions granulomateuses et l’exclusion des autres granulomatoses à discuter en fonction de la présentation.
Sarcoidosis is a granulomatous disease of unknown etiology. Lung and lymphatic system are the principal localisations. Clinical presentations are various depending on involved organs. Some presentations, which are easily diagnosed, are typical and frequent. Atypical forms have unusual presentations and/or are rare. Beside, in a multisystemic sarcoidosis, the affection of only one organ can be unusual. Rigorous diagnosis procedure could avoid errors.Twenty percent of sarcoidosis have atypical presentation. However, each of them are unfrequent. Atypical features are wide and can concern pulmonary or extrapulmonary manifestations, general manifestations, blood testing or pathological pattern.Describing atypical forms are necessary for their diagnosis. The diagnosis of atypical sarcoidosis is found on the knowledge of atypical forms previously described, presence of granulomas on specimen biopsy and excluding other granulomatous disease.
