Au cours de la renutrition, l’albumine est synthétisée comme les autres protéines. Or, les variations de l’albuminémie sont mal corrélées avec le gain d’azote. Notre hypothèse : le pool d’albumine augmente en proportion de l’azote retenu mais l’accroissement de la pression oncotique (0,75cm d’eau/g d’albumine) dilue le volume extracellulaire et atténue les variations d’ albuminémie. But du travail : estimer les variations du pool d’albumine en couplant albuminémie et l’impédance corporelle à 5kHz (Z5) et les comparer avec le bilan d’azote et la force musculaire.L’albumine est diluée dans le volume extracellulaire et le pool d’albumine=Cte×volume extracellulaire×albuminémie. Les études de dilution isotopiques ont montré que le volume extracellulaire est fonction de T2/Z5 (T=taille). Le produit albuminémie×T2/Z5 est proportionnel au pool d’albumine, à une constante près, propre à chaque patient. Ne pouvant la déterminer, on définit un indice d’albumine totale (IAT) où IAT=albuminémie×T2/Z5 et on suppose invariable la répartition de l’albumine entre les secteurs intravasculaire et interstitiel chez un même patient à 15jours d’intervalle. Les variations de l’indice d’albumine totale ne dépendent alors que de celles de l’albuminémie, dosage biologique fiable, et de celles de Z5, mesure physique des propriétés électriques du milieu extracellulaire, reproductible avec tous les impédancemètres. Nous n’avons pas utilisé les valeurs de volume extracellulaire proposées par l’impédancemètre, calculées à partir de multiples facteurs (âge, poids, taille, sexe, Z5, caractéristiques de la population témoin) selon des algorithmes complexes propres au constructeur. La force musculaire est mesurée par dynamomètre médical (Charder®), Z5 est calculée à partir de la résistance (R) et de la réactance (X) mesurées à 5kHz par impédancemètre Bioparhom® : Z5=racine (R2+X2). Population : patients admis en soins de suite pour renutrition de juin à août 2015. À j1 et j15, mesure de Z5 et de la force musculaire, dosage de l’ albuminémie. À j1, j15 et 2 fois par semaine, évaluation des ingesta protidiques (Nutriciel®), dosage de l’urée urinaire en mmol/24h (UU). Apport d’azote (AA)=moyenne des ingesta protidiques g/24h/6,25. Pertes d’azote=moyenne de UU×0,028+3g. Bilan d’azote moyen par 24h=AA–UU×0,028–3g. Statistiques : test-t de Student pour les différences de séries appariées, coefficient de corrélation.Patients : n=22, 8H, 14F, 63±21ans. Tous ont eu 5 évaluations des ingesta et 4,6±1,2 de UU. La prise de poids (2,1±6,7kg) n’est pas significative. Le gain d’albuminémie (2,38±3,4g/L) est significatif (t=3,28, p<0,01) mais n’est pas corrélé significativement à la prise de poids (r=0,17), ni au bilan d’azote (r=0,20) ni aux variations de force musculaire (r=0,27). BA n’est pas corrélé aux variations de force musculaire (r=0,06). En revanche, l’augmentation de l’indice d’albumine totale (211±207) est significative (t=4,79, p<0,001) et elle est corrélée au BA (r=0,44, p<0,05) et aux variations de FM (r=0,43, p<0,05).Les variations de l’indice d’albumine totale traduisent celles du pool d’albumine et sont mieux corrélées au bilan d’azote que celles de l’ albuminémie. L’indice d’albumine totale n’est applicable que si le patient est son propre témoin, en présumant constante la répartition de l’albumine entre les deux secteurs extracellulaires entre deux mesures. Une étude de validation est nécessaire.
