La transplantation rénale est le traitement de choix de la maladie rénale chronique stade V. Une obésité sévère constitue un frein à l’inscription sur liste d’attente. Un partenariat a été instauré entre le service de transplantation rénale et le centre spécialisé de l’obésité (CSO) du CHU de Bordeaux. Nous avons évalué l’effet de cette prise en charge sur la levée de contre-indication temporaire à la greffe.Nous avons recueilli les données concernant la maladie rénale causale, ainsi que le grade d’obésité et ses complications pour les patients adressés au CSO entre novembre 2010 et février 2016, du fait d’une contre-indication temporaire à la greffe. Un suivi pluridisciplinaire était proposé. Nous rapportons les propositions thérapeutiques, l’évolution pondérale et le statut sur liste de greffe en fin de suivi.Vingt-neuf patients ont été inclus. Deux patients ne sont pas venus. La population était composée de 68,9 % de femmes, d’âge moyen 54,17±11,4 ans. L’IMC moyen était de 39,7±5,6kg/m2 [31,6–52,3kg/m2]. Initialement, 81,5 % des patients étaient dialysés ; 81,5 % étaient hypertendus, 59,3 % étaient diabétiques de type 2. La durée moyenne du suivi était de 19,9±16 mois [2–52 mois], le pourcentage de perte de poids de 5,17±6,7 %, avec un IMC final de 37,8±5,3kg/m2. La contre-indication temporaire à la greffe a pu être levée pour 13 patients (44,4 %), dont huit ont été greffés. L’IMC moyen final de ces patients était de 34,8kg/m2. Trois patients ont bénéficié d’une plastie abdominale, afin de faciliter l’abord chirurgical, et deux patients d’une chirurgie bariatrique. Trois patients sont décédés, sans lien avec la prise en charge. En ce qui concerne les patients pour lesquels la contre-indication temporaire à la greffe était toujours en vigueur, cinq étaient en rupture de suivi, un patient a eu une contre-indication médicale définitive, cinq étaient encore en cours de prise en charge. Pour les deux patients non venus, la contre-indication temporaire à la greffe n’a pas été levée.La prise en charge multidisciplinaire au sein du CSO est efficace permettant l’accès à la greffe de 59 % des patients adhérents au programme, avec 36 % de patients greffés. Pour ceux-ci le suivi après la greffe avec le risque de reprise pondérale est essentiel.
