La prévalence de l'atopie et de ses manifestations a augmenté au cours des trente dernières années. La composante génétique de ces maladies représente certainement le facteur de risque le plus important, mais ne peut à elle seule expliquer cette progression rapide. Il existe d'importantes variations de la prévalence de l'atopie entre les Pays de l'Est et les Pays Occidentaux, entre les régions industrialisées et les régions rurales, entre les pays riches et les pays en voie de développement. Des facteurs liés aux différents aspects de la famille ou de la société permettent d'expliquer en partie ces différences. Les facteurs liés à notre mode de vie occidentale ou au niveau socio-économique élevé apparaissent impliqués dans l'expression des manifestations de l'atopie. Parmi eux, la moindre exposition des enfants aux infections au cours des premières années de vie, pourrait expliquer le déséquilibre Th2 lymphocytaire impliqué dans l'atopie. Ces constatations épidémiologiques pourront peut-être apporter de nouveaux éléments pour une meilleure prévention de l'atopie.
The prevalence of atopy and its manifestations has increased over the last thirty years. The genetic component of these diseases certainly represents the largest risk factor, but is not the only factor accounting for this rapid growth. There are marked variations in the prevalence of atopy between Eastern and Western countries, between industrialized and rural regions, between rich countries and developing countries. These differences can be partly explained by factors related to the various aspects of the family or society. Factors related to our western lifestyle or to the high standard of living appear to be involved in the expression of the manifestations of atopy. Among these factors, decreased exposure of children to infections during the first years of life could explain the Th2 lymphocyte disequilibrium involved in atopy. These epidemiological findings could possibly provide new elements to allow better prevention of atopy.
