L’ampleur de l’insuffisance rénale au cours de la maladie à fièvre hémorragique Ebola est inconnue en Guinée Forestière. L’épidémie a été déclarée urgence nationale le 13 août 2014. Malgré les efforts fournis par tous les acteurs de santé impliqués dans la Réponse Ebola, six préfectures restent actives. Notre objectif est d’identifier la prévalence et proposer les stratégies d’intervention.Nous avons mené une étude prospective au CTE Macenta en Guinée Forestière, du 26 mars 2014 au 29 novembre 2014. Le virus Ebola était isolé par le Laboratoire Pasteur en utilisant la PCR sur le plasma, la salive ou le swab. L’insuffisance rénale aiguë était définie par la classification de RIFLE. La créatinine et l’ionogramme étaient déterminés au Piccolo. Le logiciel Epi Info 7 ;1, version 14 était utilisé pour l’analyse statistique.Soixante patients ont été pris en charge au CTE Macenta. Vingt-deux (36,7 %) étaient positifs,12 (20 %) étaient décédés et 8 (13,3 %) étaient guéris. Seize (26,7 %) patients avaient présenté une IRA définie par la classification de RIFLE.L’IRA est fréquente au cours des maladies virales [1]. Elle traduit l’incapacité des reins à éliminer les déchets métaboliques et à maintenir normale l’homéostasie hydroélectrolytique. Au cours de la maladie à fièvre hémorragique à virus Ebola, l’hypoperfusion rénale est à la base de l’IRA fonctionnelle.L’origine fonctionnelle de l’IRA doit être évoquée dans 65 % des cas [2]. Cette constellation permet de considérer toute IRA au décours de la maladie hémorragique à virus Ebola comme étant d’origine fonctionnelle. Cette étude est la première menée en Afrique de l’Ouest. Elle indique un besoin de développer des stratégies basées sur la médecine individuelle dans la prise en charge globale des patients atteints de la maladie à fièvre hémorragique Ebola.
