L'osteoarthropathie hypertrophiante est une entite clinique bien connue. Les principales etiologies sont pulmonaires (cancer, infection, cardiopathie cyanogene) et gastro-intestinales (maladies inflammatoires). Les formes unilaterales localisees sont egalement classiques au decours d'osteomy elite, de syphilis, de metastases pulmonaires, de meningiome angioblastique, d'hepatopathie severe, de maladie de Crohn, de vascularite necrosante ou encore d'insuffisance veineuse chronique.Les auteurs nanceiens rapportent une observation d'osteopathie hypertrophiante revelatrice d'une infection de prothese vasculaire aortique, chez un homme de 67 ans, sans autre antecedent qu'une claudication intermittente ayant necessite un pontage aorto-bifemoral 5 ans avant. L'apparition progressive des douleurs et d'un oedeme du membre inferieur gauche conduisent a un certain nombre d'explorations complementaires, dont des radiographies osseuses. Celles-ci mettent en evidence des appositions periostees unilaterales. La scintigraphie osseuse au technetium 99m objective alors une augmentation lineaire pericorticale de la radioactivite, touchant exclusivement le membre inferieur droit (tibia, perone, femur). L'imagerie en resonance magnetique (IRM) confirme la presence d'un epanchement articulaire, d'une synovite du genou et surtout l'existence d'une importante periostose separee du cortex osseux par une ligne claire. Malheureusement, le patient refuse les explorations complementaires. Il est hospitalise en urgence 4 mois plus tard pour une ischemie aigue du membre inferieur gauche dans un contexte febrile. Les hemocultures sont positives a Streptococcus mellitus. L'infection de la prothese conduit a un pontage extra-anatomique axillo-bifemoral. La mise en culture du greffon aortique revint positive a Enterococcus et Corynebacterium verosis. Malheureusement, en depit de ce traitement chirurgical et d'une antibiotherapie adaptee, le patient decede d'un choc septique avec defaillance multiviscerale.Cette observation d'osteopathie hypertrophiante revelatrice d'une infection de materiel prothetique vasculaire est l'occasion pour les auteurs de faire le point sur cette entite peu connue, decrite pour la premiere fois en 1972 (King JO. Br Med J 1972:4:404-5). A l'exception d'une observation d'un tel tableau survenant au decours d'un pontage axilloaxillaire, toutes les observations rapportees dans la litterature surviennent au decours d'une infection d'une prothese aortique. Le delai moyen entre le geste chirurgical et la survenue de cette osteoarthropathie varie de quelques moisa 10 ans. La plupart des patients se presentent avec un tableau de douleurs progressives et diffuses de l'un ou des deux membres inferieurs associees a des epanchements articulaires du genou ou de la cheville. La presence d'orteils en massue est signalee chez pres d'un malade sur deux. Comme toujours dans cette situation, les signes infectieux sont souvent peu importants ou non specifiques. Les explorations sont dominees par la radiographie, la scintigraphie et l'IRM. Certes, les signes scintigraphiques precedent les anomalies radiographiques, mais l'examen le plus precoce et le plus specifique semble etre l'IRM, dont l'interet serait double : diagnostic de l'osteopathie hypertrophiante, exclusion d'une eventuelle osteomyelite associee ou encore d'une infection des tissus mous. La meilleure connaissance de cette entite, par les internistes, les rhumatologues, les infectiologues, les angiologues et les chirurgiens vasculaires devrait permettre d'evoquer plus rapidement le diagnostic de sepsis de materiel prothetique chez des patients ayant beneficie d'une chirurgie aortique. Cette notion est particulierement importante car le pronostic est fonction de la precocite diagnostique.
