Le syndrome de Gougerot-Sjögren primitif (SGJp) est une connectivite auto-immune associant une atteinte glandulaire à une atteinte extra-glandulaire fréquemment. Les manifestations musculosquelettiques sont communes. L’atteinte enthésitique, comme celle observée au cours des spondylarthrites, est un sujet de débat actuel.L’objectif de cette étude était de rechercher une atteinte enthésitique chez un groupe de malades suivis pour un SGJp et d’évaluer son éventuelle association avec les paramètres cliniques de la maladie.L’étude a été menée au service de rhumatologie de l’hôpital Mongi-Slim en Tunisie, entre juin 2015 et décembre 2016, incluant 25 patients atteints du SGJp. Les malades répondaient aux critères du consensus américano-européens pour le SGJp. Chez chaque patient, 12 enthèses ont été palpées cliniquement par un rhumatologue (épicondyles latéraux et médiaux, tendons quadricipitaux, tubérosités tibiales, tendons achilléens et aponévroses plantaires). Un score enthésitique clinique a été calculé. Les articulations des poignets et des mains (les cinq MCPs et IPPs) ont été examinées de façon bilatérale afin de déterminer le nombre d’articulations douloureuses (NAD) et tuméfiées (NAT). Également, les valeurs de SSDDI et ESSPRI ont été calculés.Tous les sujets inclus étaient de sexe féminin. L’âge moyen était de 53,2±11,3 ans [31 ; 70]. L’indice de masse corporelle moyen était de 29,9±6,4kg/m2 [20,2 ; 46]. La durée d’évolution moyenne de la maladie était de 4,6±4 ans [1–15]. L’ESSPRI moyen était de 19,3±5,6 [9–29]. Le SSDDI moyen était de 1,8±1,6 [1–7]. Le NAD moyen était de 1,4±3,1. Il était à zéro chez 16 patientes. Sur un total de 550 articulations examinées, huit articulations présentaient une synovite clinique (1,4 %). Il s’agissait des poignets chez une patiente et du poignet, 2e, 3e, 4e, 5e MCP et 3e IPP droits chez une autre patiente. Le score enthésitique moyen chez chaque individu était de 3,8±3,3. Nous avons trouvé une corrélation positive entre le score enthésitique clinique et ESSPRI (r=0,47 ; p=0,01) ainsi qu’avec le NAD (r=0,49 ; p=0,01). Il n’existe pas de corrélation avec le SSDDI (r=−0,16 ; p=0,42) ni le NAT (r=0,34 ; p=0,08).Notre étude montre que l’atteinte des enthèses est une source de douleur et de gêne fonctionnelle chez les patients suivis pour SGJp.
