Les medicaments a visee hemostatique font desormais partie de l'arsenal therapeutique pour la prise en charge de l'hemorragie digestive aigue d'origine haute.Chez les malades atteints de cirrhose, les vasopresseurs tels que la terlipressine et la somatostatine et ses derives ont fait la preuve de leur efficacite pour assurer l'hemostase primaire. Ils doivent etre debutes precocement, si la cirrhose est diagnostiquee, sans attendre la confirmation endoscopique des signes d'hypertension portale. Ils peuvent etre poursuivis lorsqu'une hemostase endoscopique s'est averee necessaire.Dans l'hemorragie d'origine ulcereuse, l'utilisation des inhibiteurs de la pompe a proton reduit probablement la recidive hemorragique mais leur interet en terme de mortalite n'est pas demontre. Leur efficacite maximale s'observe en cas d'ulcere a fort risque de recidive hemorragique. S'ils sont utilises, ils doivent l'etre a fortes doses et par voie intraveineuse.De nouvelles etudes doivent etre entreprises pour preciser la duree optimale d'utilisation de vasopresseurs dans l'hemorragie chez le malade atteint de cirrhose et pour confirmer l'interet clinique des antisecretoires gastriques en cas d'hemorragie ulcereuse.
Pharmacological treatments such as terlipressin or somatostatin and its analogues are effective in controlling variceal bleeding in patients with cirrhosis. They should be started immediately if cirrhosis is clinically diagnosed, without waiting for endoscopic confirmation of portal hypertension signs. They also improve the efficiency of emergency sclerotherapy.Proton pump inhibitors are probably useful to prevent recurrent bleeding in patients with peptic ulcer, but improvement on survival has not yet been shown. The main benefit is observed in case of ulcer with high risk of recurrent bleeding. If proton pump inhibitors are used, they must be administrated at high dosages and intravenously.New trials are required to determine the optimal duration of the haemostatic pharmacological treatment in patients with cirrhosis and to confirm clinical interest in the use of proton pump inhibitors in case of ulcer bleeding.
