Les antifongiques disponibles en therapeutique sont actuellement l amphotericine B ainsi que ses nouvelles formulations lipidiques, la flucytosine et les derives azoles (ketoconazole, fluconazole, itraconazole). Les cibles de ces molecules sont des composants de la membrane fongique, essentiellement l ergosterol. L etude de la sensibilite des agents pathogenes aux antifongiques est delicate et insuffisamment codifiee. Les concentrations minimales inhibitrices ont une valeur predictive faible pour le succes ou l echec clinique. La correlation entre les donnees in vitro et les resultats in vivo n'est bonne que dans le cas des candidoses oropharyngees du sujet infecte par le virus de l immunodeficience humaine. Les associations d antifongiques sont a l etude. L interet des facteurs de croissance hematopoietiques (G-CSF ou GM-CSF) comme traitement associe n'est pas formellement demontre. De nouveaux agents antifongiques (triazoles, echinocandines) devraient etre disponibles dans les prochains mois.
The available antifungal agents are amphotericin B (conventional or lipid formulation), flucytosin and azole derivatives (ketoconazole, fluconazole, itraconazole). The main target of these molecules are a specific compound of fungal membrane, ergosterol. Determination of the fungal sensitivity to antifungal drugs is difficult and no consensus has been achieved so far. Minimal inhibitory concentrations are poor predictors of clinical success or failure. A good correlation between in vitro and in vivo results has been observed only in patients with oropharyngeal candidiasis associated with HIV infection. Combinations of antifungal drugs are currently under study. The role of hemopoietic growth factors (G-CSF, GM-CSF) as an adjuvant has not been fully established. New antifungal drugs (triazole derivatives, echinocandins) should be available within months.
