Le codage des RUM est obligatoire dans le cadre du financement des Hôpitaux par la T2A. L’infectiologie est une spécialité dite étiologique, et le codage selon la CIM10 pourrait mieux rendre compte d’une activité d’organe par rapport à notre activité. En outre, améliorer ce codage dans un but d’optimisation financière de la T2A peut-il dégrader le reflet de l’activité médicale « infectiologique » d’un Service de Maladies Infectieuses et Tropicales (SMIT) au profit d’une activité dite d’organe et remettre en question, à terme la nécessité pour un Centre Hospitalier de disposer de tels services ?Le codage de l’activité médicale selon la CIM10 dans notre CHU, se fait depuis le début de l’année 2013 via le logiciel d’aide au codage appelé Web100T-Praticiens. Les données générées sont analysées avec le Logiciel PMSIPilot, utilisé par environ la moitié des CHU en France.Nous comparons les données d’activité du SMIT, secteur d’hospitalisation, du 01/01/2010 au 30/11/2013, (avant et après implémentation de l’outil web -100T) en termes de GHM, DMS, mortalité, données sociodémographiques, et T2A.Les résultats montrent une valorisation forte en terme T2A de l’activité avec une augmentation d’environ 500 000 € (+ 26 %) entre 2012 et 2013 accompagnant une augmentation de l’activité et la décentralisation du codage sans aucune dégradation de la description infectiologique par la CIM10 de l’activité du service, avec des GHM à très forte dominance infectiologique. Ainsi les infections respiratoires, ostéo-articulaires, les endocardites et le VIH sont les principaux GHM, stables en nombre malgré une optimisation du codage entre 2010 et 2013 grâce à une collaboration étroite praticiens et personnels PMSI.En conclusion, l’amélioration T2A du codage PMSI grâce à un outil web d’aide au codage n’empêche pas et surtout ne dégrade pas la description PMSI de l’activité infectiologique d’un SMIT.
