Les syndromes d'insulino-résistance extrême sont un ensemble de pathologies rares d'étiologies diverses.Nous rapportons le cas d'un diabète de nosologie indéterminée avec insulinorésistance extrême et décompensation acidocétosique survenue 10 ans après le diagnostic initial.Il s'agit d'un patient de 83 ans, normopondéral, avec des besoins insuliniques très élevés mais variables au cours du nycthémère, avec des débits insuliniques pouvant atteindre 100UI/h au pousse-seringue électrique dans la journée, contrastant avec des débits de 0,3 à 3 UI/h d'insuline la nuit avec la survenue d'hypoglycémies nocturnes. Ce syndrome d'insulino-résistance extrême était associé avec la présence d'anticorps anti-insuline à titre très élevé, supérieurs à 90 %, en l'absence d'anticorps anti-récepteur à l'insuline. Un syndrome lymphoprolifératif B à immunoglobuline M de type lymphome lymphocytique a pu être diagnostiqué. Le patient a bénéficié de 3 échanges plasmatiques et de 3 cures de chimiothérapie associant du Rituximab, du Cyclophosphamide et de la Méthylprednisolone. Ces traitements ont permis une amélioration rapide et spectaculaire de l'insulinorésistance avec un bon équilibre glycémique sous 34 UI d'insuline par jour avec en parallèle une diminution du taux des anticorps anti-insuline et une régression du pic monoclonal d'immunoglobuline M. Quatorze mois après la fin de la chimiothérapie, il a présenté une récidive du syndrome lymphoprolifératif B avec une réapparition de son insulino-résistance extrême, qui ont régressé après une nouvelle ligne de chimiothérapie.Ce cas très rare d'insulino-résistance extrême illustre la relation entre insulino-résistance, anticorps anti-insuline et hémopathie maligne. En pratique clinique, il apparaît important de contrôler les anticorps anti-insuline en cas d'apparition récente d'une insulino-résistance extrême et de rechercher, en cas de positivité, une hémopathie sous-jacente. Le traitement de l'insulinoré-sistance est lié à celui de l'hémopathie.
