The diagnosis of unexplained inflammatory syndromes requires many investigations which are commonly expensive, often invasive and must be repeated several times. A means for physicians to improve the diagnosis procedure would be to have new tests available to select the best diagnosis procedures. In order to test the value of the protein profile (PP) in the case of unexplained inflammatory syndromes, we prospectively studied 95 among 650 patients admitted in our unit for an inflammatory syndrom defined by a C Reactive Protein level > 2 mg/dL and a sedimentation rate > 30 mm, which remained unexplained after a 5 day hospitalization (49 men, 46 women; mean age: 62.9 ± 12.3 years). PP has been considered as a useful test for the diagnosis in 21 cases (22%) having been definitively established by pathologic examination (42%), radiologic procedures (24%), endoscopy (19%), immunological tests (19%), and serological test (9.5%). The definitive diagnosis was a systemic disease in 12 cases, infection (four cases), malignancy (three cases), amyloidosis (one case) and ocult intestinal bleeding (one case). Conclusion. - Our data indicate that PP is a useful test for the diagnosis of unexplained inflammatory syndromes; its main utility is the disclosure of some specific biochemical syndromes available for better selecting the definitive diagnosis procedures.Le diagnostic des syndromes inflammatoires inexpliques necessite des investigations souvent longues, couteuses et generalement repetees selon une strategie pas-a-pas. Un moyen de reduire le cheminement diagnostique serait de disposer de tests d'orientation destines a mieux selectionner les procedures diagnostiques decisives. Nous avons teste l'interet du profil proteique dans une telle indication au cours d'une etude prospective conduite de fevrier 1995 a fevrier 1996 chez 650 patients, admis pour un syndrome inflammatoire defini par une vitesse de sedimentation superieure a 30 mm a la premiere heure et un taux de proteine C reactive superieur a 20 mg/L a l'admission. Resultats. - Quatre-vingt-quinze patients (14,4 %) presentaient un syndrome inflammatoire considere comme inexplique lorsque le diagnostic restait inconnu apres 5 jours d'hospitalisation (49 hommes et 46 femmes, age moyen : 62.9 ± 12,3 ans). Le diagnostic a ete etabli ulterieurement par l'anatomopathologie dans 42 % des cas, par l'imagerie medicale dans 24 % des cas, par l'endoscopie dans 19 % des cas, par l'immunologie dans 19 % des cas et par la serologique dans 9,5 % des cas. Le profil proteique a apporte une aide au diagnostic dans 21 cas (22 %). Les diagnostics retenus onte te une maladie systemique dans 12 cas (50 %), une maladie infectieuse dans quatre cas, une neoplasie dans trois cas, puis un cas d'amylose AL et un cas de carence martiale sur saignement occulte. Conclusion. - Notre etude montre que le profil proteique est bien adapte a l'exploration des syndromes inflammatoires inexpliques. Son interet essentiel est de reveler l'existence de syndromes biochimiques specifiques, d'interet physiopathologique, utiles a la selection des procedures diagnostiques decisives.
