Explorer de nouvelles hypothèses étiopathogéniques des hernies de l'aine, par une étude biomécanique et histologique du fascia transversalis et de l'aponévrose du grand droit.Les prélèvements tissulaires ont été effectués chez 63 patients porteurs de hernie unilatérale ou bilatérale et chez 30 témoins sans hernie. Les propriétés biomécaniques ont été évaluées en utilisant un appareil de succion computérisé (Cutometer ® ). L'étude histologique a été réalisée après coloration des coupes à l'hématoxyline—éosine, au trichrome de Masson et à la double coloration associant le rouge sirius et l'orcéine.Aucune différence biomécanique et histologique importante ne fut observée entre les aponévroses des patients et des témoins. Les fascias provenant des côtés non herniés des patients présentaient une augmentation d'extensibilité et d'élasticité par rapport à ceux des témoins. Leur trame de collagène se caractérisait également par des zones structurellement désorganisées avec augmentation du nombre de fibres isolées.Le fascia transversalis provenant du côté non hernié semble correspondre à un fascia pathologique. Une pathologie du tissu conjonctif, s'exprimant préférentiellement dans la région inguinale et affectant principalement les fibres de collagène, paraît impliquée dans la genèse des hernies inguinales.
To explore new etiological hypotheses of groin hernias, using a biomechanical and histochemical study of the transversalis fascia and of the rectus sheath.The samples were collected from 63 patients with uni- or bilateral hernias and from 30 control subjects without hernia. The biomechanical properties were assessed using a computerized-suction device (Cutometer ® ). Sections were stained with hematoxylin and eosin, Masson-trichrome and double-stained with sirius red and orcein.There was no major biomechanical and histological difference between the control and patient aponeuroses. The patient fascias from the non-herniated sides presented an increased extensibility and elasticity, as compared with the control fascias. Those fascias also showed a collagen framework with disorganized areas and an increased number of isolated fibers.The transversalis fascia from the non-herniated side appears to be a pathological fascia. Therefore, a connective tissue pathology seems to be involved in the genesis of groin hernias, with a preferential manifestation in the inguinal region which affects mainly the collagen fibers.
