Zapalenia wątroby typu B i C mają istotne znaczenie ze względu na rozwój przewlekłego procesu chorobowego, marskości wątroby oraz pierwotnego raka wątroby. Jednym z leków stosowanych w terapii przewlekłych wirusowych zapaleń wątroby jest interferon α. Próby leczenia interferonem u dorosłych podjęto ponad 20 lat temu, natomiast u dzieci i młodzieży wprowadzenie terapii opóźniała obawa przed wystąpieniem objawów ubocznych. Najczęściej obserwuje się objawy grypopodobne, poza tym występują nudności, brak łaknienia, niekiedy granulocytopenia, małopłytkowość oraz niedokrwistość ustępujące po zakończeniu leczenia. Ze względu na objawy ze strony przewodu pokarmowego, nierzadko doprowadzające do ubytku masy ciała, konieczne jest monitorowanie stanu odżywienia dzieci i młodzieży. Oceny stanu odżywienia dokonuje się za pomocą wskaźników antropometrycznych oraz biochemicznych. Oznaczanie poziomów hemoglobiny, białka całkowitego oraz albumin umożliwia identyfikację chorych z upośledzonym odżywieniem.Celem pracy była ocena wybranych parametrów biochemicznych krwi jako wskaźników stanu odżywienia u dzieci i młodzieży z przewlekłym wirusowym zapaleniem wątroby poddanych monoterapii interferonem α.Analizą objęto dane zawarte w indywidualnej karcie pacjenta dotyczące 130 pacjentów w wieku od 2 do 17 lat z przewlekłym zapaleniem wątroby rozpoznanym na podstawie typowych kryteriów diagnostycznych. Stan odżywienia oceniono opierając się na wskaźnikach biochemicznych przed leczeniem, w 6., 12., 24. tygodniu terapii. Ze względu na charakter retrospektywny pracy wykorzystano dostępne dane.Na podstawie przeprowadzonych badań i analizy statystycznej stwierdzono, że terapia interferonem α dzieci i młodzieży z przewlekłym wirusowym zapaleniem wątroby wywiera niewielki i przejściowy niekorzystny wpływ na parametry biochemiczne krwi, których monitorowanie może zapobiegać pogłębianiu się zaburzeń stanu odżywienia podczas interferonoterapii.
Hepatitis B and hepatitis C are crucial with respect to the development of a chronic illness, cirrhosis of the liver and primary cancer of the liver. Interferon α is one of the drugs used in the therapy of chronic viral hepatitis. Attempts of interferon treatment in adults were made as early as 20 years ago, however, the introduction of interferon treatment in children and teenagers was delayed for fear of occurrence of side effects. Among the most commonly observed symptoms are influenza-like symptoms. Additionally, there appears nausea, lack of appetite, sometimes granulocytopenia, thrombocytopenia and anemia, which regress upon the completion of treatment. Considering the presence of symptoms from the gastrointestinal tract, frequently resulting in the loss of body mass, there must be stressed the necessity of monitoring the nutritional status of children and teenagers. The assessment of the nutritional state is carried out by means of anthropomentric and biochemical indexes, with BMI and Cole's index being the most frequently used ones. Centile charts are perfect for interpreting the spectrum of variability of both body mass and height. Determining levels of hemoglobin, total protein and albumins enables the identification of patients with impaired nutrition.The aim of the paper was to assess the nutritional status of children and teenagers with chronic hepatitis who underwent a monotherapy with interferon α.The analysis was based on the data from an individual patient's medical history of 130 patients aged from 2 to 17 years with chronic hepatitis diagnosed on the basis of standard diagnostic criteria. Their nutritional state was assessed on the basis of biochemical indexes before treatment and in 6, 12, 24 week of the therapy. Due to a retrospective character of the paper, already available data was used.On the basis of both the examinations and statistical analysis it was concluded that chronic viral hepatitis has no significant influence of biochemical parameters of blood of children and teenagers with chronic viral hepatitis B and C during monotherapy with interferon α.
