Évaluer l’efficacité et la tolérance du RAL à M6 et déterminer le profil de résistance en cas d’échec thérapeutique chez les patients VIH 1 de sous type non B.Étude pilote, ouverte, multicentrique, évaluant l’efficacité et la tolérance d’une trithérapie comprenant RAL 400mg en 2 prises/jour chez des patients naïfs de traitement, de sous type viral B et non B.63 patients sont inclus dont 40 non B, 41 hommes (83 % dans le groupe B, 55 % dans le groupe non B) et 22 femmes (respectivement 17 % et 45 %). L’âge médian est de 42,8 ans. Dans le groupe B, 35 % des patients sont hétérosexuels, 39 % homosexuels, contre 80 % et 10 % dans le non B. Le sous type CRF02-AG représente 48 % des non B.A l’inclusion la CV moyenne est de 4,8 log, IC95 (4,41–5,12) dans le groupe B, et de 4,6 log, IC95 (4,23–4,9) dans le non B. La moyenne des CD4 est respectivement pour les groupes B et non B, de 284/mm 3 , IC95 (206–361) et 316/mm 3 (269–364) et la moyenne du CD4/CD8 est de 0,30 (0,22–0,39) et 0,37 (0,29–0,44).À M6 la moyenne des CV est respectivement de 1,49 (1,33–1,65) et 1,54 (1,42–1,65) (p=0,279). La décroissance de la CV est très importante entre J0 et J14 et similaire dans les deux groupes. La moyenne des CD4 est respectivement de 415/mm 3 , IC95 (336–495) et 457 (399–513), CD4/CD8 0,48 (0,34–0,63) et 0,66 (0,52–0,80).Il n’y a pas de différence significative pour les autres paramètres de surveillance (hépatique, rénal, lipidique). Aucun échec thérapeutique n’est mis en évidence.Avec une trithérapie comprenant du Raltégravir, la décroissance de la CV est significative, surtout à J14. Une augmentation significative des CD4 et du %CD4 a été observé durant toute la durée du suivi. Il n’y a pas de différence entre les 2 groupes. L’étude est poursuivie jusqu’à M12 et M24, les résultats sont en cours d’analyse.
