La fonction rénale résiduelle (FRR) représente un impact favorable sur la qualité de vie des patients en insuffisance rénale chronique terminale. Sa préservation constitue un objectif majeur à intégrer dans la prise en charge du patient en dialyse péritonéale (DP), d’où l’intérêt d’une bonne connaissance des facteurs de risque qui influencent le déclin rapide de cette FRR, ce qui constitut l’objectif de notre travail.Étude rétrospective, menée en juin 2016, incluant 36 patients en cours de DP continue ambulatoire (DPCA), ayant un suivie minimum de 6 mois, avec une diurèse supérieure à 100mL par 24 heures, sous furosémide 500mg à 1g par jour et une FRR supérieure à 2mL/min. Nous avons calculé la vitesse de déclin de la FRR sur les six première mois de DP. Un déclin rapide est défini par une vitesse supérieure à 0,2mL/min/mois, puis nous avons réparti nos patients en deux groupes en fonction de la présence ou non d’un déclin rapide de la FFR afin d’en rechercher les facteurs prédisposant.L’âge moyen des patients est de 47,09±17 ans, avec un sex-ratio de 1,25. La néphropathie initiale est diabétique dans 26,7 %. La médiane de la durée en DP est 30,49 mois. La diurèse moyenne chez nos patients à l’admission était de 1790±528mL/j, dont 22 % été sous icodextrine. Un test de perméabilité péritonéale est réalisé chez 75 % des patients étudiés a objectivé un péritoine hyper-perméable modérée dans 50 % des cas. En analyse univariée l’âge supérieur à 50 ans, le sexe masculin et une protéinurie initiale à l’admission supérieure ou égale à 1g par 24 heures sont ressortis comme facteurs associés au déclin rapide de la FRR. En analyse multivariée, seule la protéinurie à l’admission est retenue comme facteur indépendant des autres facteurs.Le facteur de risque associé au déclin rapide de la FRR, trouvé dans notre étude est la protéinurie initiale supérieure à 1g/24h. Ce résultat rejoint la littérature d’où l’intérêt de l’utilisation des bloqueurs du système rénine-angiotensine-aldostérone. Le surpoids et l’hyperperméabilité du péritoine classiquement retrouvés comme facteurs prédictifs du déclin de la FRR, ne sont pas ressortit comme facteurs de risque.La préservation de la FRR est un élément important à intégrer dans la prise en charge des patients en DP, afin d’assurer une dialyse adéquate et une meilleure qualité de vie.
