Descrivere i processi molecolari e fisiologici alla base del funzionamento del sistema endocannabinoide, analizzare le diverse tipologie di approccio farmacologico mirate alla sua modulazione e stabilire le implicazioni di tale sistema nella patogenesi di alcune patologie di pertinenza odontostomatologica.Sono stati selezionati e analizzati gli studi della letteratura internazionale inerenti la fisiologia e la farmacodinamica degli endocannabinoidi e di molecole di sintesi a essi affini. Sono state inoltre riportate le più significative evidenze sperimentali relative alle potenzialità terapeutiche di tali sostanze nell’ambito della clinica odontoiatrica e stomatologica, come la cura e la prevenzione della malattia parodontale e la chirurgia implantare.La modulazione farmacologica del sistema endocannabinoide e di altri sistemi neuroendocrini a esso correlati può indurre effetti terapeutici di particolare interesse, anche nell’ambito della stomatologia e della chirurgia orale, come l’analgesia, l’azione antinfiammatoria e la modulazione, sistemica e localizzata, del metabolismo osseo. L’applicazione clinica di tali strumenti farmacologici risulterà tanto più ampia quanto maggiore sarà la loro capacità di evitare gli effetti indesiderati derivanti dall’attivazione dei recettori cannabinoidi.
To describe the physiological and molecular processes involved in the endocannabinoid system, to analyze the different types of pharmacological approaches used to modulate its function, and to examine the implications of this system in the pathogenesis of several oral pathologies.We selected and analyzed relevant papers from the international literature on the physiology and pharmacodynamics of endocannabinoids and their synthetic analogues. This review examines the most significant experimental data regarding the therapeutic potential of these substances in clinical dentistry and stomatology (e.g., in the treatment and prevention of periodontal disease and in oral implant surgery).Pharmacological modulation of the endocannabinoid system and other related neuroendocrine systems can produce interesting therapeutic effects in dentistry and oral surgery, including analgesia, anti-inflammatory actions, and systemic and localized modulation of bone metabolism. The breadth of their clinical applications depends largely on the prevention of the side effects related to cannabinoid receptor activation.
