Dans le cadre de la mise en place d’une politique de substitution, un registre a été mis en place pour évaluer la tolérance et l’efficacité d’un biosimilaire de l’infliximab dans des indications hors autorisation de mise sur le marché (AMM) dans un service de médecine interne.Il s’agissait d’une étude de cohorte prospective observationnelle. Tous les patients du service de médecine interne traités par le biosimilaire d’infliximab Inflectra® (en initiation de traitement ou en relais de traitement par infliximab–Remicade®) pour des indications hors AMM ont été inclus dans l’étude après consentement. Le critère principal de jugement était le nombre d’effets indésirables attribués au traitement au cours du suivi. Les critères secondaires incluaient : les rechutes de la maladie, la survenue d’infections sévères, le nombre d’interruption de traitements.Trente-six patients ont été inclus dans le registre, 14 hommes et 22 femmes d’âge médian à l’inclusion de 45 ans [19–72]. Quinze patients étaient naïfs de traitement par infliximab (42 %) et 21 patients ont switché vers le biosimilaire en cours de traitement (58 %). L’indication du traitement était une sarcoïdose pour 17 patients (47 %), uvéite non sarcoïdose non Behçet pour 6 patients (17 %), uvéite postérieure de la maladie de Behçet pour 5 patients (13 %), maladie de Takayasu et maladie de Behçet pour 1 patient chacune (3 et 3 %), autres indications pour 6 patients (17 %). Six patients étaient diabétiques (17 %). L’évolution médiane de la maladie au moment de l’inclusion était de 5 ans [1–48]. La durée médiane de suivi a été de 4 mois [de 1 à 9]. Trois réactions à type de malaise, une réaction allergique cutanée et une réaction au point d’injection ont été notées. Onze patients ont eu une infection au cours du suivi (31 %) dont 2 ont nécessité une hospitalisation. L’intervalle médian entre deux cures d’infliximab était de 4 semaines [4–8]. Trente-deux patients avaient une dose de 5mg/kg, 1 une dose de 3mg/kg et 3 une dose supérieure à 5mg/kg. Neuf patients ont rechuté sous biosimilaire (25 %) dont 8 étaient déjà traités initialement par infliximab (soit 89 %). Pour 3 patients, le traitement a été interrompu : 1 pour inefficacité, 1 pour effet indésirable et 1 pour les 2. Tous les patients étaient traités concomitamment par corticostéroïdes avec une posologie moyenne quotidienne de prednisone de 11,1±6,3mg à la 1re perfusion de biosimilaire d’infliximab et de 8,9±4,0mg à la dernière visite. Vingt-quatre patients étaient traités concomitamment par méthotrexate (67 %) et 6 par azathioprine (17 %). Pour les 21 patients antérieurement traités par Remicade®, la durée médiane du traitement par infliximab avant inclusion était de 18 mois [2–57], l’intercure médiane était de 5 semaines [4–8]. Parmi ces patients, 3 ont présenté des réactions lors d’une perfusion (2 malaises et 1 réaction cutanée). Sept ont présenté une infection dont 1 grave ayant nécessité une hospitalisation. Aucun patient inclus dans le registre n’a présenté de lymphome, maladie auto-immune ou maladie démyélinisante apparu(e) en cours de traitement par infliximab.Les biosimilaires sont des biothérapies qui contiennent une version d’une substance active qui a déjà reçu une autorisation de mise sur le marché. Les producteurs sont tenus de montrer leur équivalence en terme de caractéristiques physicochimiques, effets indésirables et efficacité. Les biothérapies anti-TNF-alpha sont des traitements largement utilisés dans le monde et ayant une AMM dans la polyarthrite rhumatoïde et les maladies inflammatoires du tube digestif. Malgré l’utilisation croissante des biosimilaires, des questions persistent sur leur efficacité, tolérance et en particulier leur immunogénicité. En médecine interne où l’infliximab est fréquemment utilisé en dehors des indications de l’AMM, aucune donnée n’est disponible sur l’efficacité et la tolérance de l’infliximab. Dans ce registre, 14 % des patients recevant un biosimilaire de l’infliximab ont présenté des effets indésirables lors d’une injection et le traitement par Remicade® a été repris chez 2 sans réapparition des effets indésirables. Par ailleurs, 25 % des patients ont présenté une rechute de leur maladie et il s’agissait principalement de patients antérieurement traités par Remicade® (8/9). Les doses administrées de même que le phénotype de la maladie (présence d’atteintes neurologiques nécessitant de plus fortes doses) ne sont pas comparables aux patients des indications de l’AMM. Même si les données recueillies dans cette étude ne permettent pas de conclure formellement en l’absence de bras contrôle, la substitution chez ces patients doit être envisagée avec prudence.
