We report a case in which a patient with dilated cardiomyopathy presented with syncope, terminated by a shock from his implantable cardioverter defibrillator. However, subsequent interrogation of the device revealed no tachycardia detection or treatment parameters. The mystifying details of the case were unravelled by remote consultation with the staff electrophysiologist and the use of smart phone-transmitted live images. This case highlights the use of mobile phone-facilitated video conferencing in urgent management of intracardiac device therapy. Judicious use of this technology has the potential to deliver effective and cost-effective solutions for many device-related emergencies in patients at remote settings.
Nous rapportons un cas où un patient ayant une cardiomyopathie dilatée a subi une syncope, qui s’est terminée par un choc de son défibrillateur implantable. Cependant, l’interrogation subséquente du dispositif n’a révélé ni détection de tachycardie ni paramètres de traitement. Les détails trompeurs du cas ont été éclaircis par la consultation à distance avec l’électrophysiologiste et l’utilisation d’images en direct transmises par téléphone intelligent. Ce cas met l’accent sur l’utilisation de vidéoconférences au moyen d’un téléphone mobile lors de prise en charge urgente de traitement par dispositif intracardiaque. L’utilisation judicieuse de cette technologie a le potentiel d’offrir des solutions efficaces et rentables pour plusieurs urgences liées au dispositif dans le cadre de la surveillance à distance des patients.
