Malgré la forte prévalence de la dénutrition à l’hôpital et les nombreux risques associés, le dépistage et la prise en charge restent insuffisants. Une prise en charge multidisciplinaire est nécessaire, incluant les médecins. L’objectif de cette étude était de décrire la prise en charge hospitalière de la dénutrition proposée par les médecins, leurs représentations et mettre en évidence les freins afin d’aboutir à des pistes d’amélioration de la prise en charge.Étude descriptive qualitative par entretiens semi-dirigés auprès de neuf médecins exerçant au centre hospitalier de Dunkerque, d’avril à août 2015. Retranscription des entretiens sur le logiciel de traitement de texte Word®. Analyse informatique des entretiens par théorisation ancrée approchée avec double codage à l’aide du logiciel NVivo10® permettant l’émergence de codes, regroupés en thèmes.L’analyse des entretiens a fait apparaître un intérêt médical de la prise en charge de la dénutrition, une bonne connaissance des complications liées à la dénutrition et des patients à risque. Beaucoup de médecins ont exprimé des difficultés à dépister la dénutrition et parfois à la prendre en charge, notamment concernant la prescription ou la gestion des complications. Les autres difficultés rapportées étaient un manque de connaissances, de réévaluation de l’efficacité de la prise en charge. Les médecins ont également rapporté des réticences médicales, paramédicales et de la part des patients concernant la prise en charge de la dénutrition. En majorité, les médecins ont précisé trouver une aide auprès de l’équipe de nutrition. Certains ont dit souhaiter des formations sur la dénutrition, d’autres la mise en place de « référents » en dénutrition dans chaque service de soins. D’autres encore ont dit vouloir un accompagnement plus important de la part de l’équipe de nutrition.La mise en place de formations, la nomination de « référents » en dénutrition dans les services de soins et un accompagnement plus important par l’équipe de nutrition pourraient répondre aux attentes des médecins. Cela pourrait permettre d’améliorer le dépistage et la prise en charge de la dénutrition à l’hôpital et d’en réduire les complications.
