Étudier la validité de deux autoquestionnaires d’activité de la polyarthrite rhumatoïde (PR), le RAPID3 et le RADAI5, en pratique quotidienne, en comparaison avec les DAS28, CDAI et SDAI. Définir des seuils de poussée de la polyarthrite, selon l’avis du patient et du médecin.Deux cents patients PR consécutifs ont rempli les questionnaires RAPID3 et RADAI5. Les indices DAS28, CDAI et SDAI ont été calculés. Le patient et le médecin se sont prononcés, de façon binaire (oui ou non), quant à l’existence d’une poussée. Le coefficient de Spearman a été utilisé pour calculer la corrélation entre les scores et indices, 2 à 2. Le coefficient Kappa a été calculé pour évaluer la concordance entre l’avis du patient et du médecin concernant la poussée. Des seuils de poussée ont été recherchés à l’aide de courbes ROC.Les 200 patients, d’âge moyen de 57ans±11,5, ayant une PR évoluant depuis 13ans±8,3, avaient en moyenne un DAS28 à 3,61±1,43, un CDAI à 12,7±9,89 et un SDAI à 13,4±10,45. La moyenne des scores RAPID3 et RADAI5 était respectivement de 3,45±2, et 3,93±2,18. Le RAPID3 et le RADAI5 sont corrélés de manière significative aux indices composites d’activité : ρ entre 0,64 et 0,74. Parmi les 200 patients, 35 % se déclarent en poussée, alors que le médecin retient une poussée dans 22 % des cas. La concordance entre l’avis du médecin et du malade sur la poussée est modérée (κ=0,44). Des seuils de poussée ont été obtenus avec une sensibilité et une spécificité satisfaisantes pour les différents scores et indices. Les seuils de poussée selon le médecin se situent dans la catégorie d’activité modérée pour les indices composites : 4,04 pour le DAS28, 14,5 pour le CDAI et 16,7 pour le SDAI. Les seuils de poussée du RAPID3 et du RADAI5, selon l’avis du médecin, d’une part, et selon l’avis du patient, d’autre part, sont proches : respectivement 4,27 et 4,33 pour le RAPID3, et 4,5 et 4,7 pour le RADAI5.Ces résultats confirment la validité des autoquestionnaires RAPID3 et RADAI5 et incitent à leur utilisation régulière en pratique quotidienne chez nos patients PR. Les scores des autoquestionnaires sont corrélés aux indices d’activité composites et permettent de détecter des pics d’activité ou poussées.
