Le benefice de la prescription de betabloquants dans le postinfarctus represente une des meilleures actions de prevention secondaire scientifiquement provees. Dans cette indication, les betabloquants diminuent la mortalite et le risque de recidive de 20 a 40% selon lese tudes, qui ont inclus au total plus de 20 000 patients. Neanmoins, la pratique courante prouve qu'une partie des survivants d'un infarctus ne recoivent pas ce traitement au profit de classes therapeutiques (les inhibiteurs calciques) n'ayant pas demontre de benefice dans cette indication. Les sujets ages sont les plus souvent plus particulierement prives de la prescription des betabloquants par crainte des prescripteurs de troubles de la conduction, ou de mauvaise tolerance hemodynamique. L'objectif de cettee tude americaine a ete de demontrer d'une part la sous-utilisation des betabloquants chez le sujet age de plus de 65 ans et, d'autre part, de prouver que cette attitude largement repandue est deletere pour le patient.Il s'agit d'une etude retrospective ayant inclus 5 332 patients ages de plus de 65 ans, survivants a 1 mois d'un infarctus du myocarde. En respectant les contre-indications retenues par les auteurs (insuffisance cardiaque congestive, asthme, diabete insulinorequerant), 3 737 patients etaient eligibles pour le traitement par betabloquants. Vingt et un pour cent de ces patients recevaient un traitement betabloquant a la sortie de l'hopital. Les deux facteurs predictifs de la non-utilisation des betabloquants etaient l'age avance et la prescription d'inhibiteurs calciques. Apres ajustements aux autres facteurs connus de mortalite, le taux de mortalite chez les patients traites par betabloquants etait inferieur de 43 % a celui observe chez les patients sans betabloquants (RR : 0,57 ; 0,47-0,69). L'effet benefique sur la mortalite etait observe dans toutes les sous-classes d'age, et en particulier audela de 85 ans (601 patients). De plus, les patients traites par betabloquants etaient moins souvent rehospitalises (RR : 0,78 ; 0,67-0,90). Le traitement par inhibiteurs calciques prescrit au lieu du traitement betabloquant etait associea un doublement du risque de mortalite (RR : 1,98 ; 1,44-2,72), non par un role deletere des inhibiteurs calciques mais par la non-prescription des betabloquants.Cette etude souligne l'interet des betabloquants dans le postinfarctus, et elle demontre la persistance de l'effet benefique dans des classes d'age peu ou pas explorees dans les grands essais therapeutiques des annees 1980. La crainte des prescripteurs ne se justifie pas, meme si la posologie et la surveillance de ce type de traitement doivent etre adaptees aux caracteristiques du sujeta ge. La substitution dans l'esprit et dans la pratique d'une frange encore trop importante des medecins des betabloquants par des inhibiteurs calciques prouve une fois encore son inefficacite en prevention secondaire de l'infarctus du myocarde.
