L'application de l'impedancemetrie bioelectrique (IB) a l'evaluation du statut hydrique des enfants en insuffisance renale terminale (IRT) pourrait etre d'un grand interet pratique. Toutefois, les equations validees chez l'enfant sain (Davies, 1988) ne sont pas forcement applicables dans cette situation pathologique, en raison des variations electrolytiques susceptibles de modifier la conduction du courant electrique dans l'organisme. L'objectif de ce travail etait d'etudier la sensibilite de l'IB multifrequence aux modifications de l'hydratation chez des enfants et adolescents en IRT, au cours des seances d'hemodialyse. Population. Il s'agit de sept patients (quatre filles), ages de 5 a 20 ans (dont six de plus de 14 ans), en IRT et en hemodialyse chronique avec ultrafiltration. Methode. Chez chaque patient, au cours d'une seance d'hemodialyse, ont ete realisees quatre mesures d'impedance (Analycor 2 , Eugedia, quatre electrodes collees) a 5,50 et 100 kHz, permettant de calculer l'indice de resistance (IR = T 2 [cm 2 ] / resistance [Ohm]) avant, pendant et apres la seance d'hemodialyse. Les variations de l'IR aux trois frequences, pendant la seance d'hemodialyse, ont ete correlees au volume d'eau ultrafiltree entre chaque mesure d'impedance. Resultats. Il existait une correlation significative entre l'IR et l'eau ultrafiltree aux differentes frequences; c'est avec les mesures realisees a une frequence de 50 kHz que la correlation etait la plus etroite. L'equation de regression lineaire etait la suivante : volume (L) = 0,45 Δ.IR + 0,138 (r = 0,92; p < 0,001). Par ailleurs, les volumes calcules par l'equation de Davies etaient, chez nos patients, significativement differents des volumes d'ultrafiltration mesures (+ 360 mL ; test t apparie ; p < 0,0035). Au total. L'IB a 50 kHz est une technique simple et fiable pour mesurer l'eau corporelle chez les enfants en IRT. Elle necessite cependant l'utilisation d'equations specifiques a cette pathologie.
