Le déficit immunitaire commun variable, le plus fréquent des déficits immunitaires symptomatiques de l’adulte, est défini par une hypogammaglobulinémie primitive (par opposition à secondaire). C’est un syndrome hétérogène, associant au plan clinique de façon presque constante des infections bactériennes à répétition, des maladies auto-immunes (d’organe ou contre les lignées hématopoïétiques), un syndrome lymphoprolifératif (hyperplasie folliculaire, maladie granulomateuse). La gravité des infections et leur répétition doit faire évoquer le diagnostic, qui sera confirmé par une électrophorèse des protides et un dosage pondéral des Ig. Le traitement repose sur la substitution régulière en Immunoglobulines polyvalentes par voie intraveineuse ou sous-cutanée, qui permet de diminuer la fréquence des infections des voies aériennes, et probablement d’éviter leur complication (dilatation des bronches). La physiopathologie de cette maladie est très mal comprise, mais la description récente de défauts génétiques chez de rares malades devrait permettre d’en améliorer la compréhension.
Common variable immunodeficiency is the most frequent symptomatic immunedeficiency of adulthood. It is defined by primary (opposite to secondary) hypogammaglobulinemia. It's an heterogeneous disorder, that can include repeated bacterial infections, and/or auto-immune and/or lymphoproliferative diseases. The diagnosis is suspected if infectious diseases are serious and/or unusually frequent, and it has to be confirmed by gammaglobulins and/or IgG, A and M dosage. Treatment is based on immunoglobulin replacement, which allow to drastically diminish the frequency and severity of infectious bacterial diseases. The pathophysiology of this rare disease is poorly understood, but the recent description of molecular defects in rare genetic diseases will maybe help us to understand the mechanisms of hypogammaglobulinemia in these patients.
