La transmission du parasite Toxoplasma gondii de la mère à l'enfant s'effectue par l'intermédiaire du placenta qui constitue un réservoir de parasites. Un des moyens de diagnostic de la toxoplasmose congénitale à la période néonatale est donc constitué par l'isolement du parasite par placentoculture et inoculation du caillot du sang de cordon à l'animal. Ces examens ont montré une sensibilité respectivement de 94,28 % et de 61,46 % dans notre expérience sur 95 placentas et 57 sangs de cordon inoculés. Le titrage des IgG ne constitue aucun argument précoce de diagnostic puisque les IgG maternelles transferrent la barrière placentaire. Ainsi le suivi sérologique doit être pratiqué pendant 12 mois pour confirmer ou exclure le diagnostic de toxoplasmose congénitale. La mise en évidence d'IgM par une technique d'immunocapture et d'IgA par un test ELISA ont une sensibilité de 85,2 % et 82,1 % sur 61 sangs de cordon étudiés. Ces résultats peuvent être améliorés par l'application du Western-Blot au diagnostic néonatal de la toxoplasmose congénitale constituant un moyen précoce de diagnostic. Cet examen doit être renouvelé au cours des 3 premiers mois de vie en particulier chez les enfants dont la mère a présenté une séroconversion au cours du 1er trimestre ou au cours du 9 e mois de la grossesse.
The transmission of Toxoplasma gondii from mother to baby was made by placenta that is a reserve of parasites. At the birth, parasites were inoculated from placenta (94.28%) and cord blood (61.46%) from inoculated 95 placenta and 57 cord bloods. The IgG detection was not an early diagnostic sign. These IgG were maternal IgG. Blood samples were regularly taken from the babies during the first year to confirm or exclude the diagnosis of congenital toxoplasmosis. The IgM by immunocapture technic and IgA by ELISA were respectively positives in 82.5% and 82.1%. The sensibility of these results can be increased by Western Blot application at the birth. Western-Blot must be follow-up during the first three months of life, specially in case of seroconversion mother during the first trimester or the ninth month.
